- Mississippi Job Corps Center (MsJCC) is soliciting bids for eye exam services and prescription eyewear services for students of the Mississippi Job Corps Center, Crystal Springs. All interested parties should submit a pricing program and proposal with a written response via email (rdionne@del-jen.com) or by fax, 601-892-6956, or by USPS or other acceptable delivery methods, by the close of business day on July 22, 2009. The successful bidder must comply with all Federal Acquisition Regulation clauses, be compliant under all related federal statutory requirements and regulations, and provide evidence of licenses/certificates/insurance required for eye exams and eye wear dispensing. Scope of Work 1. Provide eye exam services 2. Dispense basic prescription eye wear (single lens, non tinted, non polarized) including a frame, for students attending classes at Treasure Island Job Corps Center. 3. The location(s) must be conveniently located in the Crystal Springs/Jackson area (within a 50 mile radius of Crystal Springs), and the hours of operation must be include business days and one weekend day. Specifications Provide a flat fee for eye exams, one per student per year Provide an inclusive flat fee for a limited selection/choice of male and female frames, single lens, non tinted, non polarized, one per student per year Invoice Mississippi Job Corps Center, once a month, with a detailed billing Provide a quote for Tinted lens on an as approved basis by MsJCC Accept Medical – This is an option; not accepting Medical will not disqualify your bid.
