Description
Declaration for Federal Employment Declaration for Federal Employment SF 1449 Temporary Waiver of Character Investigation This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation is issued as Request for Quotes (RFQ) #246-09-Q-0029. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33, effective 06/15/2009. This procurement is a 100% small business set-aside under NAICS code 621399 with a standard business size of $7 million. Contractor shall provide a firm-fixed price daily rate (all inclusive to include travel and per diem) for 120 eight-hour Dental Hygiene clinics at the Wewoka Indian Health Center, July 27, 2009 to July 26, 2010. Description of Requirements: The purpose of this request is to provide eligible Indian patients if the Wewoka PHS Indian Health Center, dental hygiene services via 8-hour clinics conducted at that facility as requested and scheduled by the Chief, Dental Services. The contract will run for one (1) calendar year beginning on July 27, 2009 through July 26, 2010. The Contractor shall provide dental hygiene services which will expand present services that are not available to all adults seeking these services. The Contractor shall advise on or administer and perform dental hygiene services at chair side, include primarily educational and therapeutic preventive periodontal services and secondarily, routine dental prophylaxis. The Contractor shall provide consultation to the Wewoka PHS Indian Health Center dental staff in the prevention and treatment of periodontal diseases through personal contact. Services are provided in a clinical environment under quality management of the Chief, Dental Services of the Wewoka PHS Indian Health Center or his designee. Service priorities are determined primarily by the dental public health philosophies and available resources and secondarily, by individual patient needs. The Government shall furnish the necessary facilities, equipment, instruments and materials for the Contractor to perform radiographs, examinations, prophylaxis, root planing and preventive oral health education and services at the Wewoka PHS Indian Health Center. DENTAL HYGIENE SERVICES 1. The contractor will make available the personnel to provide 120 eight-hour clinics. 2. The Contractor shall abide by all policies and procedures of the Indian Health Services and the Wewoka Service Unit. 3. The Contractor shall be subject to DHHS established standards of conduct. 4. The Contractor shall provide patient education and instruction to patients, parents and guardians including but not limited to information on dental disease processes and the principles of oral health disease prevention and instructions on proper methods of oral health care that are appropriate to the patient's age and needs. 5. The Contractor shall provide periodontal services appropriate to the age and needs of the patient including but not limited to: a) Root planing b) Curettage c) Periodontal charting and other diagnostic procedures including radiographs d) Coronal polishing 6. The Contractor is expected to arrive at the clinic no later than 8:00 am on clinic days in order that treatment may be initiated on the 8:00 am patients as close to the appointment time as possible. 7. The Contractor is expected to maximize time management and efficiency of hygiene and infection control procedures so that no less than 8 patients are seen and appropriately treated during the clinic day. 8. The Contractor shall be responsible for sharpening and maintenance of instruments and equipment used in patient treatment. 9. The Contractor shall be responsible for maintenance and cleaning of the dental unit at the end of each work day. 10. The Contractor shall be responsible for restocking of supplies used in the operatory at the end of each work day. 11. The Contractor shall expose and process radiographs and shall assist with performing oral examinations on recall patients if time and workload permit. 12. The Contractor shall assist the receptionist, when necessary, with the hygiene recall system and the contacting of short notice patients for treatment. 13. The Contractor shall not engage in personal business during clinic hours. 14. The Contractor shall meet the following minimum professional qualifications and requirements: a) Completion of a course of study in dental hygiene accredited by the American Dental Association as evidenced by a degree or certification. b) A valid current and unrestricted license to practice dental hygiene in a State of the United States of America. 15. The schedule will consist of 120 outpatient dental clinics. A typical clinic shall consist of an eight hour shift, beginning at 8:00 am and ending at 5:00 pm, with a lunch period from 12:00 pm to1:00 pm. Half days may be scheduled, with pro-rated payment, by special arrangement with the Chief, Dental Services. Work availability schedules will be confirmed at least one month in advance. NON-PERSONAL HEALTH CARE SERVICE CONTRACT: This contract is a non-personal health care service contract as defined in 37.101 under which the contractor is an independent contractor. This professional service of the contractor will be evaluated by the Government via routine IHS staff members review of physicians charts on a quarterly basis or thereafter as appropriate, however, the Government retains no control over the medical professional aspects of service rendered. The provisions of 52.212-1, Instructions to Offerors-Commercial Items (June 2008), applies to this acquisition. In addition to the information required in FAR 52.212-1, Contractors shall provide the following: (1) completed "Temporary Waiver of Character Investigation" and "Declaration for Federal Employment" form (2) Dun and Bradstreet Number (note: Contractors must be registered in the Central Contractor Registration to be eligible for award. This can be done at http://www.ccr.gov). The provisions of 52.212-2, Evaluation - Commercial Items (Jan 1999) applies to this acquisition. The Government intends to award one from this solicitation to the responsible offeror(s) whose offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate quotes: (1) Price; (2) Qualifications. Both factors are equal in importance.
