Description
STATEMENT OF WORK I Scope of Work The contractor shall provide all management, supervision, labor and shall plan, schedule, coordinate and assure effective performance of all cooks and line servers described herein at a fixed price set forth in Contractor's Quote. The contractor will be required to provide line servers and cooks services in accordance with the requirements of the Combat Readiness Training Center (CRTC), to support the deployment of the 138th Fighter Wing. II Services Required The contractor shall provide food services as follows: The contractor will provide individuals to prepare serve and replenish meals to personnel from the 138th FW and units involved in their deployment 7-16 August 2009. Specific meal time will be provided to those parties that respond to this Notice. The service will include replenishing the salad bar and beverages. The contractor will be required to keep the prep area and serving line clean and after the meals to include all equipment used to prepare the meals i.e. stoves, ranges, and all preparation equipment. One serving line will be utilized during serving hours. III General The contractor shall arrange for satisfactory supervision of the contract workers. The contractor's supervisor shall be available at all times when the contract work is in progress to receive notices, reports, and/or request from the government. IV Qualifications of Personnel A. All Supervisory personnel engaged in directing the work to be accomplished under the contract shall possess at least five (5) years of recent Military Food Service experience in directing Food Service Operations in a Supervisory capacity in dining hall similar to the one where services are to be provided. B. All serving personnel mush have at least two (2) years food service experience. These determinations should be made by the contractor and may be made by the 138th Services personnel who are appointed to oversee this contract. The names of these individuals will be provided to the contractor upon arrival on 10 August 2009. C. All supervisory personnel engaged in directing the work to be accomplished under this contract shall possess at least two (2) years of recent (within the past 5 years) experience in directing Food Service Operations in a supervisory capacity for the Dining Hall in this contract. D. A list of personnel that will be servers and cooks will be required, along with a letter of their experience covering their qualifications. At least two (2) years of experience will be required for the cooks. All personnel of the contractor, working under this contractor will have a copy of a Food Handling Permit with the supervisor on site at all times. E. The contractor will furnish personnel to perform head count functions. Follow store room procedures IAW AFI 34-239. The contract will complete the following AF Forms: AF Form 1650 Daily Dining Facility Summary, AF Form 148 Senior Cooks Requisition, AF Form 3516 Food Service Inventory Transfer Receipt when required. V Government Furnished Supplies, Materials, Equipment and Utilities The Government will furnish all power, gas, water, cleaning supplies, equipment, utensils and subsistence to perform the requirements of this contract. VI Quality Control Program The contractor shall establish and maintain quality control programs for the purpose of tracking and enforcing the requirements of this contract. One copy of the contractor's quality control plan shall be provided with the submission of the proposal to this contract. The plan shall primarily include but not be limited to inspection system covering all the items started in the Services required and Quality sections of this contract. This inspection system will include at a minimum checklist, which will ensure continuity for a 24 hour operation and a system for correcting and identifying deficiencies. The plan will be reviewed by the contracting officer prior to award of the contract to ensure adequacy. All checklists and inspection reports will be kept at the contract site during the term of the contact for inspection by the contracting officer or the identified Services personnel upon request. These documents will also be turned in with the invoice to the Services QAP (QAP). Due to the lack of deployed contracting personnel, the 138th FSV QAP can modify the meal hours if necessary due to any mission situations. Most importantly the system will focus on achieving quality rather than looking for mistakes.
