- Statement of Work 1.This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Food and Drug Administration intends to award a competitive purchase order for Attending Veterinary Services for the White Oak, Maryland Animal Program. 2.Request for Quote (RFQ) # 1059513. 3.The RFQ document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 05-30. 4.This requirement is for full and open competition under NAICS 541940 “Veterinary Services” ($6.5 million size standard). 5.In order to appropriately perform this task, the veterinarian must be licensed and have obtained the status Diplomate, ACLAM, at least 10 years of experience. 6.FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. 7.FAR 52.212-2, Evaluation – Commercial Items, applies to this acquisition.. 8.Offerors should include a copy of the provision FAR 52.212-3, Offeror Representations and Certifications – Commercial Items with their quote. 9.FAR 52.212-4, Contract Terms and conditions – Commercial Items applies to this acquisition. 10.FAR 52.212-5, Contract Terms and conditions Required to Implement Statues or Executive Orders – Commercial Items applies to this acquisition.
