- Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents.The Richard L Roudebush VA Medical Center (RLR VAMC), Indianapolis, IN, is conducting a market survey for qualified firms capable of providing the required sharps collection and removal services outlined below. Applicable NAICS code is 562119 size standard $12.5 million. Responses to this notice must be submitted in writing (email or fax is acceptable) and received no later than July 15th, 2009. No telephone inquires will be accepted. Description of work to be performed includes: The Contractor shall provide service to collect, dispose, sanitize and replace reusable sharps containers throughout the Richard L. Roudebush VA Medical Center, located at 1481 W. 10th Street, Indianapolis, IN Contractor shall provide resistant sharps containers for lease and provide for a one-day per week, or as needed, regular collection and disposal of sharps waste from the premises based upon location of containers. Contractor shall sanitize the reusable sharps through an EPA approved process to prevent cross contamination. "Sharps Waste" shall include hypodermic needles, syringes, scalpel blades and other sharp items. The term does not include any hazardous waste, radioactive waste, pharmaceuticals. The Medical Center will be responsible for the segregating of all waste in accordance with Federal, State and Local requirements. The Contractor shall warrant and represent that all personnel assigned to the premises under this contract are their employees and have been trained in the performance of the duties set forth in this contract, in accordance with Federal, State and Local Regulations. The Contractor agrees to comply with the policies of the Medical Center to include compliance with the standards of the JCAHO. The contractor will ensure strict confidentiality of all sensitive material which includes but not limited to, patient and staff information. Contractor will not be required to accept any hazardous chemical waste, pharmaceutical, or radioactive waste. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, HubZone, 8(a), small disadvantaged business, small business, or large business) relative to NAICS 562119 (size standard of $12.5 million per annum). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, a service capability statement, examples of similar facilities to which you have provided similar services to, DUNS Number, address, point of contact, and the size of your business pursuant to the following questions: (1) Is your business large or small? (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified HubZone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business?
