- SF18 Document Solicitation (includes Statement of Work and Confidentiality Statement) The Health Resources and Services Administration (HRSA) is soliciting proposals in accordance with the Simplified Acquisition Procedures in Part 13 of the Federal Acquisition Regulations (FAR). Award will be limited to Small Business concerns. The purpose of this procurement is to produce an annual white paper on key findings and the state of agricultural health and safety issues in rural America. The contractor shall identify and work with four (4) States to serve as pilot sites for creating networks of providers to focus on agricultural health and safety. The contractor shall work with community entities to share information and train community-based health care providers on how to assess risk factors on farms so as to reduce the rate of illness and injury. This procurement shall be processed in accordance with FAR Part 13, Simplified Acquisitions. The period of performance for this project will be one (1) twelve (12) month base period plus one (1) 12-month option period. The solicitation is attached to this web posting. All questions about this requirement must be submitted by email to kantwi@hrsa.gov no later than 10AM Eastern Standard Time (EST) on Monday, July 13, 2009. Responses to questions will be posted at this site. The closing date for receipt of proposals will be on Monday, July 20, 2009, at no later than 4PM Eastern Standard Time (EST).
