Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation W91YU0-09-T-0051 of Purchase Request W68MX391494853 is issued as a request for quotation (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33. The associated North American Industrial Classification System (NAICS) code for this procurement is 334510. The Size Standard for small business is 500 employees. This RFQ is being issued as UNRESTRICTED. These items are for use at Madigan Army Medical Center and the proposed requirement is for a one-time buy. All responsible Contractors shall provide an offer for the following Brand Name or Equal Products: The Salient Characteristics that an or equal product must meet to be considered: Micromaxx high resolution, all digital 8-pound ultrasound system with 10.4 inch Digital display. Standard features include display module, 2D grayscale with SonoMB compound imaging and Auto Gainon selected transducers, M-mode, HIPPA compliance tool set, comprehensive, application-specific calculation packages, pc direct connectivity capability(w/SiteLink & USB Cable ordered separately). Also include batter, ultrasound gel and user guides. Line Item 0001: Ultrasound System MicroMaxx 3.4.5 Line Item 0002: Critical Care/ICU (DICOM) Package Line Item 0003: 8-4 MHZ Tranducer, Biopsy Compatible Line Item 0004: 13-6 MHZ Transducer Line Item 0005: Mobile Docking System Enhanced (MDSE) Line Item 0006: Sony UP 897 MD Black & White Video Printer Line Item 0007: Triple Transducer Connect (TTC) w/Quick Disconnect Line Item 0008: 60 Month Warranty Delivery shall be FOB Destination to Madigan Army Medical Center. Pricing quoted shall include all applicable fees. The following FAR clauses and provisions in their latest editions apply to this combined synopsis/solicitation.
