- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a solicitation W91YU0-09-T-0052 of Purchase Request W68MX391476189 is issued as a Request for Proposal (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-33. The associated North American Industrial Classification System (NAICS) code for this procurement is 423450. Size Standard for small business is 500 employees. RFQ is being issued as UNRESTRICTED. These items/supplies are for use at the Department of OBGYN, Madigan Army Medical Center and the proposed contract is for a one-time buy. The Salient Characteristics that and or equal product must meet to be considered: The system must have Dorado KT, Reverse Compatible Software that ensures the patient data can be read and retrieved for retrospective analysis and playback, optional radio frequency remote, voice annotation prompting (GPS for urodynamics), it must be able to have optional HL7 Interface for military EMR, intergate with the optional TETRA NIRS (Near Inrared Spectroscopy) system that analyzes bladder activity using non-invasive laser patch technology. It must have the ability to add on the optional NUWAV USB connected handheld ultrasound for anal sphincter 360 deg scanning and data storage with patient records. All responsible Contractors shall provide an offer for the following: LINE ITEM 0001: Dorado Kit Urodynamic 1 EA LINE ITEM 0002: Shipping Charges 1 EA LINE ITEM 0003: Trade-In Old System 1 EA LINE ITEM 0004: Arm 1 EA LINE ITEM 0005: Vaginal Probe 1 EA LINE ITEM 0006: Anal Probe with Software 1 EA Delivery shall be FOB Destination to Madigan Army Medical Center. Pricing quoted shall include all applicable fees.
