Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation M6700109Q0235 is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-33. This combined synopsis/request for quote will be on an unrestricted basis. The North American Industrial Classification System (NAICS) code for this requirement is 611699 and the small business size standard is $7.0M. The following Federal Acquisition Regulation (FAR) and Defense Federal Acquisition Regulation Supplement (DFARS) provisions and clauses apply to this acquisition: FAR 52.212-1 Instructions to Offers--Commercial Items; FAR 52.212-2 Evaluation-Commercial Items (basis for award is best value, evation factors are depth of class content, scope of class, training location(closer to Camp Lejeune the better), class commencment date, and cost; FAR 52.212-3 Offeror Representations and Certifications--Commercial Items, Certifications shall be submitted with quotation; FAR 52.212-4 Contract Terms and Conditions--Commercial Items; FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes of Executive Orders--Commercial Items with the following applicable clauses 16,17,18,19,21,31; FAR 52.204-7 Central Contractor Registration; FAR 52.222-50 Combating Trafficking in Persons; DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders! Applicable to Defense Acquisitions of Commercial Items; DFARS 252.225-7000 Buy American Act-Balance of Payments Program Certificate; DFARS 252.225-7001 Buy American Act and Balance of Payments Program; DFARS 252.212-7000 Offeror Representations and Certifications-Commercial Items; and DFARS 252.232-7003 Electronic Submission of Payment Requests. The provisions and clauses can be found at FAR website: http://www.arnet.gov/far/. Clin 0001: Advanced Medic Training is needed for 28 United States Navy Hospital Corpsman, the training is desired to take place 20 July - 24 July 2009. The following criteria is required but not limited to:Regional and dental anesthesia and emergencies, with associated lab; and regional anesthesia of orthopaedic wounds: The current standard for battlefield injury anesthesia is IM Morphine. The side effects of morphine render a patient ineffective and dangerous. In combination with hemostatic agents and pressure dressings if only part of the limb is affected, regional anesthesia for face and extremity wounds would enable a Marine to continue to function in combat. The lab shall focus on students performing regional anesthesia on each other.-Prolonged management of chest tubes with suction, insertion of nasogastric tubes, and urinary catheterization are all valuable skills involved in the management of the critically injured patient facing a prolonged medical evacuation. This shall be emphasized in a live-tissue lab.-Mass Casualty triage focusing on the squad or platoon Corpsman as the senior triage officer with Combat Life Saver, DMOC, or untrained Marines to assist him mimics a very real likelihood of a small group faced with an IED blast or ambush. This should be emphasized in a live-tissue lab.-Advanced wound debridement and suturing techniques, to include absorbable and "deep" This shall be included in a lab, and repeated in a live-tissue lab.- Diagnosis of orthopaedic injuries and treatment of somatic dysfunction with basic manipulation skills in the field. Diagnosis, taping, casting and manipulation labs shall incorporate students performing all of the above on each other. -"Tricks of the Trade" training that incorporates lessons learned and offers practical battlefield common skills. Examples would be arranging medical bag items for easy access for locating in a dark or battlefield location. Shall include a lab for the exercise. Live-tissue training will be a significant part of the above proposed course. All interested parties shall have the proper accredidation to be able to perform live tissue training. All vendors shall submit quotes in accordance with the guidelines set forth in the Policy Guidance for Combat Trauma Training That Includes Live Tissue Training, signed by Lt Gen Amos (Attachment 0001). Qoutes shall include scope of classes, depth of class content, proposed training location, and class commencment dates. Qoutes shall also submit all proper certification required in order to conduct the medical training, to include the authrization from the Director, Health Servicces Training and Education Center of Excellence and a Representative of Bureau of Medicine and Surgery Veterinarian Office. The Offeror is required to provide their Contractors' DUNS Number and Commercial and Government Entity (CAGE) code with their quote. The CAGE code entered must be for that name and address provided by the Contractor. Offerors are required to be registered in the Central Contractor Registration database (CCR), and Wide Area Work Flow database. Basis for Award will be Best Value, Evation factors are depth of class content, scope of class, training location(closer to Camp Lejeune the better), class commencment date, and cost.
