Description
GENERAL The Environmental Protection Agency is requesting quotes for a Service Maintenance Agreement for software and scanners identified in Attachment 1 of this solicitation. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation is a request for quote. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-35. This solicitation is a total set-aside for small businesses associated with NAICS code 443120 - Computer and Software Stores, with a size standard of $9.0 Million. Place of delivery and acceptance is: EPA Region 1 1 Congress Street Boston, MA 02114 Place of delivery and acceptance is anticipated to change in the Year 2009 to: 94 Devonshire Boston, MA Period of performance is from 10/1/2009 - 9/30/2010. The EPA will award a purchase order contingent on availability of funds. INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS (FAR 52.212-1) (JUNE 2008) This clause applies to this solicitation and is incorporated into this solicitation by reference. An addendum to this clause is included in this solicitation. ADDENDUM to FAR 52.212-1 Offerors shall prepare their quote in accordance with FAR 52.212-1 - Instructions to Offers - Commercial Items and the following additional instructions. The following Service Contract Act Wage Determination is applicable to this solicitation and resulting contract: SCA Wage Determination2005-2255 Revision No. 10, Dated 05/26/2009. This wage determination will be incorporated into the resulting contract. Offerors shall provide the following to be considered: 1. The information identified in FAR 52.212-1 Instructions to Offerors - Commercial Items, and the additional information in this Addendum to FAR 52.212-1. 2. A firm fixed price quote for each of the items Attachment 1. Provide one quote to reflect a month by month agreement for up to 12 months and another to reflect a one year agreement. All prices shall include labor, parts, travel, and upgrade fees/licenses. 3. References for 3 recent contracts or clients (within the past three (3) years) for similar work performed. Provide client names, contact names and phone numbers. 4. A statement that states the Offeror shall hold the prices in its offer firm for 45 days from the date specified for receipt of offers. 5. A completed copy of the provision FAR 52.212-3 Offer Representation and Certification for Commercial Items or a statement that the offeror is current in the online representatives and certifications at https://orca.bpn.gov/. Any questions regarding this solicitation shall be in writing via Email to bagocius.maureen@epa.gov and ohalloran.jennifer@epa.gov no later than 12:00pm July 27, 2009. Offerors shall send quotes via Email to bagocius.maureen@epa.gov and ohalloran.jennifer@epa.gov no later than 3:00 p.m. EST on August 5, 2009. Offerors shall be registered in the Central Contractor Registration by the date specified for receipt of offers. Information on the CCR can be found in FAR 52.212-1 - Instructions to Offers - Commercial Items. The Government may contact Offeror's to clarify information to better understand their quotes or to request information that appears to be inadvertently omitted. The Government will award a contract resulting from this solicitation to the responsible Offeror that offers the best value considering past performance and price. Per the instructions, the Offeror shall submit a completed representation and certification in their offer. Ascent Capture software Qty Description of Service 1 ea Annual Maintenance, Support, and Upgrade Fee - KOFAX Full Station software license: Covers license number MF42526 for the period 10/01/2009 through 9/30/2010. 5 ea Software Maintenance Support, and Upgrade Fee - KOFAX Ascent Capture Scan Station version 7.5 software license (limited to scan no more than 5,000 pages/each month): Covers the software license of software system with serial number MF42526 for the period from 10/01/2009 through 9/30/2010. 1 ea Annual Maintenance, Support and Upgrade Fee - KOFAX Ascent Capture Scan Station version 7.5 software license (limited to scan no more than 100,000 pages/each month): Covers license with serial number MF42526 for the period 10/01/2009 through 9/30/2010. 1 ea Annual Maintenance Support, and Upgrade Fee - KOFAX Text Searchable PDF Generator version 7.5 Software license: 100,000 pages/each month. Vendor will provide first level support for software maintenance, upgrade, and support agreement. Scanner service Qty Description of Service 1 ea Provide service on one scanner certified Fujitsu 5750C Scan Care Plus including parts, labor, travel expenses, and two preventative maintenance services provided by a Fujitsu certified technician. Maintenance agreement covers period from 10/01/2009 through 9/30/2010 for scanner with serial number 77955. 2 ea Fujitsu certified BASIC PLUS POST WARANTY scanner support series contract (1-year) for a mid-volume scanner M4099 series with next business day response-time. Includes onsite resolution of scanner problems including parts, labor and travel expenses. Maintenance agreement covers the scanners with serial numbers 501622 and 501653 for the period 10/01/2009 through 9/30/2010.
