Attachment 4 - Electroninc Funds Transfer Form Attachment 3- Government Furnished Drawings Attachment 2- Aluminum Heating Boiler Specification Attachment 1- Statement of Work Solicitation The Department of Homeland Security, Federal Emergency Management Agency (FEMA), Center for Domestic Preparedness (CDP) located in Anniston, Alabama has a requirement for the demolition and removal of current boiler, associated piping, equipment, boiler stack and for the purchase and installation of replacement condensing boilers, boiler lead lag controller, positive pressure boiler vent, in-line pumps and associated piping and equipment in building 251 of the 900 Area dorm facilities. Contractor shall provide all labor, materials, tools, equipment, transportation and supervision necessary to complete the project. Contractor shall be a licensed General Contractor (GC) in the state of Alabama. Contractor must have a minimum of five (5) years of significant supplier and installer experience in commercial applications. The work includes: 1. Demolish and remove current boiler in B251, boiler room; 2. Demolish and remove current boiler associated piping and equipment that will be replaced by new boiler system; 3. Demolish and remove current boiler stack; 4. Install and set new condensing boilers in B251, existing boiler's location; 5. Provide and install new 2" insulation for new boiler associated piping; 6. Tie-in new boiler piping into existing water piping; 7. Tie-in new boiler piping into existing gas service; 8. Tie-in new controller into existing CDP electrical service; 9. Utilize existing B251 system pump; 10. Install and run a common stack header out of the existing breaching; 11. Seal roof after installing stack header flashing; 12. Provide and install new electric 3-way valve w/temperature controller for existing domestic hot water tank in dorm buildings 171 and 277 mechanical room; 13. Perform leak check on new boiler, valves and piping; 14. Test and verify operation of new boilers and controls; 15. Provide training to CDP Operations and Maintenance contractor HVAC personnel on new boiler, controller, piping and associated equipment; 16. Clean work area and dispose of old boiler, piping, electrical, etc.... A pre-proposal conference will not be held. A formal site visit is anticipated on August 13, 2009. The proposed procurement is a 100% small business set aside under NAICS 238990 with a corresponding small business size standard of $13.0 Mil. This procurement will be solicited in accordance with Sealed Bidding of FAR Part 14. The IFB will be available electronically at the Federal Business Opportunities web site, https://fbo.gov, on or about July 23, 2009 ; bids will be due August 28, 2009. Contract award date is anticipated to be on or about September 1, 2009 and work to begin approximately September 8, 2009. The contract will be awarded as a firm fixed price contract. Magnitude of this project is between $95,000.00 and $115,000.00. Parties interested in the solicitation should access the Federal Business Opportunities web site ( https://fbo.gov ) for issuance of the solicitation, associated documents and solicitation amendments; contractors will not be directly notified of solicitation amendments. All Offerors must be registered in the Central Contractors Registration database (CCR) prior to any subsequent award. Failure to be actively registered in CCR will result in disqualification for contract award.
