- PERFORMANCE-BASED WORK STATEMENT FOR HEADQUARTERS AIR FORCE RESERVE COMMAND DIGITAL DENTAL RADIOGRAPHY SYSTEM (DDRS) DATE PREPARED: 20 MAY 2009 REVISED: 24 JULY 2009 1.0 DESCRIPTION OF SERVICES. This performance-based work statement (PWS) outlines the tasks and duties required of the contractor to install equipment upgrades and personnel to provide System Administrator duties, tier I, II, and III support for the Digital Dental Radiography System (DDRS). 2.0 SERVICE SUMMARY: This Services Summary (SS) documents the services and support to be provided by the contractor. SS Performance Objectives and Performance Thresholds Performance Objective PWS Para Performance Threshold Methods of Surveillance The contractor will ensure access to DDRS 4.1.3 Accessible 24 hours each day, 7 days each week, 99% of the time Customer's feedback and quarterly review of records The contractor shall provide the results of all studies and analyses to the government. 4.2.1 Documentation shall be submitted to the Government within 30 days after requested by the Government. 100% of the time Customer's feedback and quarterly review of records The contractor shall deliver a Quality Control Plan. 4.2.1 30 days after the start date of the contract. 100% of the time 1 time review of documentation The contractor shall report all inspection results, maintenance actions, losses, and damage. 4.1.3 4.2.1 Documentation shall be provided to the Government within 30 days of action. 100% of the time Customer's feedback and quarterly review of records The Contractor shall provide information relating to the Monthly Status Reports to the Government. 4.2.2 Documentation shall be submitted to the Government on the 15th of each month. 100% of the time Quarterly review of documentation The contractor shall provide commercial computer software licenses and/or documentation. 4.2.3 Documentation and computer software licenses will be delivered to the Government within 30 days after contract award. 1 time review of documentation Travel will be in accordance with the Joint Travel Regulations. 4.14 Documentation of all contract related travel shall be approved in advance by the Government COR. 100% of the time Quarterly review of documentation Contractor complies with all security requirements in accordance with the PWS. 4.19 No tolerance for incidents of not complying with security requirements. 100% of the time Semi-annual review of records The contractor must provide verification of personnel security clearances to the Government Security Manager. 4.19.1 Documentation shall be submitted to the Government no later than 30 days after award of contract. Any changes shall be submitted to the government within 30 days of action or request from the Government. 100% of the time Customer's feedback and semi-annual review of records 3.0 GOVERNMENT FURNISHED PROPERTY AND SERVICES: 3.1 The government will provide the contractor access to systems that interface with the DDRS system. 3.2 The government will provide workspace products, such as a office workspace, chairs, microcomputers, fax, email, internet connectivity, telephone access, and routine office supplies. 3.3 The government will provide remote computer access for the contractor's personnel to provide support when on call. 4.0 GENERAL INFORMATION: 4.1 Specific Requirements: 4.1.1 The contractor will provide network engineering and professional services to install upgrades to the existing Digital Dental Radiography (DDRS) system. The contractor shall also provide personnel to support the DDRS system. The AFR has a requirement for a DDR Solution which is designed to the same specifications and uses the same software that the active duty Air Force is deploying. The Air Force DDRS is built around the MIPACS Dental Enterprise Viewer and MIPACS Storage Server. MIPACS is a commercially available Dental Imaging application with support for a broad range of image capture devices. The MIPACS Dental Enterprise Viewer is installed on a workstation or laptop and is used to capture, view, annotate and enhance dental x-ray images. The MIPACS Storage Server Software and Microsoft SQL Server Software are installed on a Microsoft Windows server to store the approved images. Microsoft SQL is used to maintain a database of patients and their stored images for easy query and retrieval. 4.1.2 Contractor Installation Responsibilities: • Responsible for ordering hardware and software required to upgrade DDRS • Responsible for providing engineering and professional services to install the DDRS upgrades • Responsible for providing professional services to test and deploy DDRS upgrades • Provide project management services to monitor and control all technical and administrative activities related to the project • Responsible for preparing a deployment schedule and implementation plan • Arrange for the procurement and shipment of the upgrade equipment to the site in advance of the installation • Update the system configuration and Department of Defense Information Awareness Certification and Accreditation Process (DIACAP) certification to reflect enhancements • Provide end user training on the use of the newly added hardware and software 4.1.3 Contractor Personnel Responsibilities: • Responsible for maintaining the servers and central archive in the remote Network Operation Security Center (NOSC); an enterprise wide application used by internal and external Air Force Reserve personnel for the Digital Dental Radiography System (DDRS) • Serves as the direct contact for Air Force Reserve dental clinic technology support technicians on all DDRS troubleshooting issues. • Responsible for creating/implementing backup and disaster recovery strategies for the DDRS system. • Provides system maintenance on the DDRS system to include firmware, driver, application software upgrades, and system health checks. • Directly interacts with DDRS vendors as required to perform troubleshooting for Air Force Reserve units experiencing DDRS problems. • Maintains both direct (phone) and indirect (email) contact with Air Force Reserve (AFR) systems personnel to assist in troubleshooting and maintaining a MiPACS server environment for AFR DICOM images. • Creates reports for DICOM image storage statistics to be viewed by management to determine usage of the AFR sites. • Proactively monitors and tunes DDRS system performance • Ensures that DDRS tickets and phone calls are handled within appropriate service level agreement timeframes • Administers the SQL Server database in a clustered environment • Directly supports all DDRS Certification and Accreditation requirement efforts • Ensures appropriate security controls to protect from unauthorized access, theft, monitoring or tampering of hardware components and/or stored data • Maintains compliance with HIPAA policies and procedures as stored data may be sensitive or confidential patient information • Works with AFRC/A6 to resolve and/or troubleshoot issues that affect application access or performance issues that related to the performance of the underlying hardware supporting DDRS, the network backbone, or other services that support the application Maintains a cohesive relationship with AFRC/A6 to ensure they have completed lists of any OS/Hardware configuration changes for DDRS system. • Keep records of inspections which will be made available to the COR electronically throughout the contract performance period and through final settlement of any claims under this contract Skill Requirements a. Minimum of 5 years of experience as a Systems Administrator b. Network Security Plus Certification c. Practical Experience and advanced knowledge of Windows 2003 and MS SQL Server d. Well versed in Active Directory at an enterprise level and Storage Area Network technologies (EMC preferred) e. Familiar with Systems Management Server (SMS) and Windows Server Update Services (WSUS) or other automated patch management applications f. Practical experience with the Air Force Digital Dental Radiography Images environment. Able to understand and apply DoD/AF instructions in relation to Time Compliance Network Order and security patching of vulnerable machines. g. Expertise in Medicor MiPACS Server required h. Skilled in backup and disaster recovery strategies i. Be able to perform detailed activities, essential tasks, and have knowledge of responsibilities that pertain to, but are not limited to, the following: 1. Managing, administering, and supporting the Customer's servers, and providing system/user support. 2. Analyzing current system setup and recommending improvements. j. Must be able to operate with minimal supervision. 4.2 Deliverables: The contractor shall provide the following deliverables to the contracting officer representative (COR) unless otherwise agreed upon: 4.2.1 Quality Control Plan (QCP): The contractor shall establish and maintain an active Quality Control Plan. Within 30 days after award of contract, the contractor shall submit a Quality Control Plan to the Contracting Officer. The Government will have 30 days to accept, conditionally accept, or reject the plan. The contractor shall update the plan whenever significant changes are needed and submit to the Contracting Officer. The times stated above for resubmittal and acceptance, etc, shall be the same. The QCP shall include a Customer Satisfaction Measurement Plan. The plan shall demonstrate the contractor's ability to measure customer satisfaction using Point of Service Surveys and Statistically Valid Random Sampling for each performance objective specified in the Service Summary. The plan shall clearly articulate how the contractor proposes to address: services that will be surveyed in a general population customer survey; services that will be administered under a point of service survey; survey tools and process (e-mail, paper, telephone or other); random sampling methodology; sample size determination; survey distribution methods; definition of survey scale and "acceptable" performance threshold; and methods of preparation, coordination, reviewing, distribution, and retention of reports. 4.2.2 Status Report: The contractor shall prepare and submit a monthly status report to include project, event, description, and next action required to stay on schedule. Submit to the COR no later than 15th day of the following month. 4.2.3 Computer Software and Computer Software Documentation: The contractor shall provide commercial computer software and/or computer software documentation acquired under licenses customarily provided to the public to the extent that such licenses are consistent with Federal law and otherwise satisfies the government needs. The contractor will not be required to furnish technical information related to commercial computer software or commercial computer software documentation that is not customarily provided to the public. The contractor will not be required to relinquish to, or otherwise provide, the government rights to use, modify, reproduce, release, perform, display or disclose commercial computer software or commercial computer software documentation. The government shall have only those rights specified in the licenses. 4.2.4 The contractor shall provide quarterly reports to identify the tasks and costs accomplished by subcontractors in support of this requirement in order to ensure compliance with FAR 52.219-14, Limitations on Subcontracting, paragraph (b)(2), Services. 4.3 Contractor Personnel 4.3.1 The contractor shall not employ individuals identified to the contractor by the CO as potential threats to the health, safety, security, or general well being, of the population, installation, or operational mission. The contractor's employees shall not pose an actual or potential threat or risk to the security or mission of the AFRC or its supported activities, or the health and safety of the AFRC workforce. The government has an inherent right under law, practice, and regulation to restrict and control access to its facilities, including those that are the subject of this SOW. The government's exercise of its right to grant and revoke access to particular individuals to its facilities shall not constitute a breach or change to this SOW, regardless of whether said individuals are employed by the contractor. The contractor shall maintain procedures for the immediate removal of employees for misconduct or other causes prejudicial to the maintenance of health, welfare, morale or security of the facilities, populace, and computer system information. The contractor shall exercise these provisions when notified by the CO that action is necessary. The contractor shall also comply with the Department of Defense (DoD) smoking policy, AFI 40-102, Tobacco Use in the Air Force, and Air Force Reserve Command (AFRC) smoking policies within DoD facilities. 4.3.2 The contractor personnel shall present a neat appearance, and be easily recognizable as contractor employees. The contractor shall provide all contractor employees with an identifying badge, which shall include as a minimum, the person's name, picture, and the contractor's name. While on the installation, the contractor employees shall wear this picture identification at all times in a clearly visible manner. The contractor personnel "signature blocks" on email and correspondence shall be structured to clearly indicate that an individual is contractor. 4.4 Information Awareness Program and Network Access Requirements: The onsite contractor personnel shall be considered users of Department of Defense (DoD) Information Systems (IS) and are required to complete initial Information Awareness (IA) orientation and annual IA refresher training as a condition of access to an IS. 4.5 Network User Licensing Program: All contractor personnel shall meet the same licensing requirements in accordance with AFI 33-115, Vol II, before access is permitted to network resources. 4.6 Network Professional Certification Program: All contractor personnel shall meet the network professional certification requirements in accordance with AFI 33-115, Vol II. 4.7 Facility Access: Access to the military installation is granted to contractor personnel only in the performance of their duties in support of this contract. Employees who have day-to-day, direct customer contact shall be able to read, clearly write, speak, and understand the English language. The contractor shall establish standards for dress, appearance, and conduct of their personnel consistent to the government's standards within the same office environment that the contracted employee will be working in. 4.8 Employment Policy: The contractor shall not employ any person who is an employee of the Air Force unless such person seeks and receives approval from the Air Force. Off-duty military personnel hired under this contract may be subject to permanent reassignment or short notice military deployment. Military Reservists and National Guard members may be subject to recall to active duty. The abrupt absence of these personnel could adversely affect the contractor's ability to successfully perform; however, their absence at any time shall not constitute an excuse for nonperformance under this contract. 4.9 Customer Complaints: The contractor shall ensure that a customer complaint process is established and advertised to system users. The contractor shall track, report, and inform the COR of the resolution of customer complaints. 4.10 Operations and Maintenance (O&M): The contractor is responsible for the operation and maintenance of the LOD system, which includes: a. End User Support -Application Administration -Tier 3 Support b. Security Management c. System Engineering Support d. Project Management Coordinator 4.11 Place of Performance: The DDRS system administrator will be performed his/her duties on site at the office of the Command Surgeon (SG), bldg 221, Robins AFB, GA. 4.12 Period of Performance: The base period of performance for this effort is date of award through one year (12 months). This effort includes four one-year (12-month) option periods, which may be unilaterally exercised by the government. Each option period shall not exceed one year in duration, unless mutually agreed upon through contract modification. All terms and conditions applicable to the base period shall extend to the option periods unless otherwise agreed upon. 4.13 Hours of Work: Unless otherwise specified by the AFRC or Site Manager, normal work hours for on-site contractor personnel will be from 0700 hrs to 1600 hrs, Monday through Friday, excluding government holidays and closure days. There will be occasions when time will need to be flexed due to units working weekends. 4.14 Travel: Travel, with estimated costs, will be approved in advance thereof by the COR through the contracting officer. The government will reimburse government approved travel and per diem costs in accordance with the Joint Federal Travel Regulation. 4.15 Training: The contractor is responsible for providing a fully trained and qualified workforce. Except as otherwise indicated, the contractor shall provide, at its expense, any required personnel training, including, but not limited to, the cost of instruction, per diem, travel, salaries, and benefits. If the contractor-identified training is available only from government sources, the contractor shall submit a written request to the COR for approval prior to each event. The request must include the training requirements with appropriate justification and estimated costs. Training and course materials received shall become the property of the government. 4.16 Conferences, Meetings and Seminars: The government will provide for technical training for newly installed or upgraded equipment and systems when approved by the COR. The contractor may be required to attend military or civilian IT conferences, meetings, and training seminars that are a benefit to the government. The contractor may be required to present briefings and/or demonstrations. 4.17 Software/Hardware Parts & Materials: From time to time, the contractor may be required to purchase software or hardware parts/materials needed in support of these contract requirements. Purchases of items not in direct support of this contract are prohibited. The COR will pre-approve all purchases valued at $2500 or more. 4.18 Safety Orientation for Work on a Government Installation: The on-site contractor personnel are required to attend a government-provided safety orientation briefing. The Government on site work center supervisor will ensure briefing is provided. 4.19 Security Requirements: The contractor shall comply with all security regulations and directives as identified herein and other security requirements as shown elsewhere in this contract. The contractor shall comply with DD Form 254, DoD Contract Security Classification Specification and Visitor's Group Security Agreement, attached to this contract (See attachment 1). 4.19.1 All on site contractor personnel shall comply with Computer Security, Emission Security, Communications Security, and Security Awareness Training and Education programs when using government provided IT/CCS equipment. Every individual who has access to the military computer network domain, specialized systems, and mission systems is a network user. Before becoming a network user, an individual must have a favorable background investigation and security clearance, be trained, and licensed. Upon verification of security clearance and successful completion of appropriate training, the user is licensed to use the network and granted access to network resources. This process of training and licensing ensures every network user is trained and aware of the basic principles of network security and their role in IA. 4.19.2 Facility Clearances and Employee Clearance: The contractor shall possess or obtain a facility clearance at the classification level of SECRET. Contract personnel are required to possess a minimum of a SECRET clearance with a privilege access of NACLC as mandated in DoD 5200.2-R, para C3.6.15 which defines the type of investigation required by each ADP (IT) level. Per DoDI 8500.2, Table E3T.1., page 45 (civilian, military, and contractors) who require IA Administrator (with no IA Administrative Privileges) access to perform their duties must be coded as IT-II and therefore meet investigative requirements of National Agency Check with Local Agency Check (NACLC). 4.19.3 The contractor shall obtain personnel security clearances on all employees who use unclassified automated and classified information systems that have access to sensitive information (IT/AIS Level 2), and access to the base network within 15 calendar days after receipt of the facility clearance or 60 days prior to performance start date if the contractor possesses a facility clearance. The government will conduct and assume all costs for personnel security investigation. 4.19.4 Contractor employees that require access to classified information or materials shall (1) possess a valid and appropriate security clearance, (2) have executed an SF 312 (Non-disclosure agreement), and (3) have a valid need for access to the information to perform a lawful and authorized government function. This is in accordance with the performance of work under this contract and shall be authorized access to classified information or material. 4.19.5 Safeguarding Classified Information: The contractor shall conform to the provisions of DOD 5220.22M, National Industrial Security Program Operating Manual (NISPOM), DoD 5220.22-R, Department of Defense Industrial Security Program, DoD 5200.1-R, Department of Defense Information Security Program and AFI 31-401, Air Force Information Security Program, and AFI 31-601 - Industrial Security Program Management. 4.19.6 Visitor Group Security Agreement (VGSA): The contractor shall enter into a visitor group security agreement with HQ AFRC in accordance with the attached VGSA format. 4.19.7 Physical Security: The contractor shall safeguard all government property IAW AFI 31-101, The AF Installation Security Program, Chapters 18 thru 26, and controlled forms provided for contractor use. The contractor shall immediately report all thefts, vandalism, or destruction of property and equipment (government or contractor owned) to the HQ SG Directorate Security Manager or Alternate Security Manager. 4.19.8 Privacy Act: Access to information subject to the provisions of the Privacy Act is not normally required; however, it is required when diagnosing and repairing site software. Personnel shall adhere to the Privacy Act, Title 5 of the U.S. Code, Section 552a and applicable agency rules and regulations. 4.20 Performance of Service During Crisis: The FC/FD has determined that this requirement is not Mission Essential (M-E) in accordance with DoDI 3020.37. 4.21 Government Holidays: The following government holidays are normally observed by government personnel: New Year's Day 1 January Labor Day 1st Monday in Sep Martin Luther King's Birthday 3rd Monday in Jan Columbus Day 2nd Monday in Oct President's Day 3rd Monday in Feb Veterans Day 11 Nov Memorial Day Last Monday in May Thanksgiving Day 4th Thursday in Nov Independence Day 4 July Christmas Day 25 Dec and any other day designated by Federal Statute, Executive Order, and/or Presidential Proclamation.
