Description
Performance Work Statement Air Force District of Washington Medical Operations AST Branch (AFDW/A7KM-S) Contracting Office intends to award a Firm Fixed Price contract for a broad range of professional Assistance and Advisory Services (A&AS). This is a combined Synopsis/Solicitation F1J4AK9176A901 for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Air Force District of Washington Medical Operations AST Branch (AFDW/A7KM-S) Contracting Office intends to award one Firm Fixed Price contract. This non-personal professional requirement provides for implementation of Outside the Continental United States (OCONUS) one-day clinical training workshops, Continental United States (CONUS) one-day workshops for military Mental Health Provider (MHP), which include psychologists, psychiatrists, social workers, mental health nurses, and mental health technicians. The goal of these workshops is to train military mental health clinicians to provide conjoint therapy for Post Traumatic Stress Disorder (PTSD) treatment for active duty military members and their significant others with combat-related stress reactions and PTSD. In addition, the programs will be evaluated to monitor training effectiveness (e.g., pre/post testing for knowledge). The contractor will provide their own equipment for the presentations, including a laptop and projector for running slides, as well as audio support, wires and cables as needed to put on a workshop as described above. The contractors chosen for the delivery of these services will have had experience treating Operation Iraqi Freedom/Operation Enduring Freedom (OIF/OEF) veterans and their families. The contractor shall furnish all labor and support required to perform the effort. The contractor must possess the specific technical skills and knowledge along with operational expertise and experience as described in the attached Performance Work Statement (PWS). NAICS Code for this synopsis/solicitation is: 611710, with a small business size standard of $7 million. All proposals will be evaluated based on FAR 15.101-2 -- Lowest Price Technically Acceptable Source Selection Process. Evaluation may be made with or without discussions with the offeror(s). The NAICS code and small business size standard for this acquisition is 611710 with a small business size standard of $7 million. (b) Submission of offers. Submit signed and dated offers to the office specified in this solicitation at or before the exact time specified in this solicitation. Offers may be submitted on the SF 1449, letterhead stationery, or as otherwise specified in the solicitation. As a minimum, offers must show -- (1) The solicitation number; (2) The time specified in the solicitation for receipt of offers; (3) The name, address, and telephone number of the offeror; (4) A technical description of the items being offered in sufficient detail to evaluate compliance with the requirements in the solicitation. Offers that fail to furnish required representations or information, or reject the terms and conditions of the solicitation may be excluded from consideration. (c) Period for acceptance of offers. The offeror agrees to hold the prices in its offer firm for 30 calendar days from the date specified for receipt of offers, unless another time period is specified in an addendum to the solicitation. (d) Product samples. When required by the solicitation, product samples shall be submitted at or prior to the time specified for receipt of offers. Unless otherwise specified in this solicitation, these samples shall be submitted at no expense to the Government, and returned at the sender's request and expense, unless they are destroyed during preaward testing. (e) Multiple offers. Offerors are encouraged to submit multiple offers presenting alternative terms and conditions or commercial items for satisfying the requirements of this solicitation. Each offer submitted will be evaluated separately. (f) Late submissions, modifications, revisions, and withdrawals of offers. (1) Offerors are responsible for submitting offers, and any modifications, revisions, or withdrawals, so as to reach the Government office designated in the solicitation by the time specified in the solicitation. If no time is specified in the solicitation, the time for receipt is 3:00 p.m., local time, for the designated Government office on the date that offers or revisions are due. (2) (i) Any offer, modification, revision, or withdrawal of an offer received at the Government office designated in the solicitation after the exact time specified for receipt of offers is "late" and will not be considered unless it is received before award is made, the Contracting Officer determines that accepting the late offer would not unduly delay the acquisition; and- (A) If it was transmitted through an electronic commerce method authorized by the solicitation, it was received at the initial point of entry to the Government infrastructure not later than 5:00 p.m. one working day prior to the date specified for receipt of offers; or (B) There is acceptable evidence to establish that it was received at the Government installation designated for receipt of offers and was under the Government's control prior to the time set for receipt of offers; or (C) If this solicitation is a request for proposals, it was the only proposal received. (ii) However, a late modification of an otherwise successful offer, that makes its terms more favorable to the Government, will be considered at any time it is received and may be accepted. (3) Acceptable evidence to establish the time of receipt at the Government installation includes the time/date stamp of that installation on the offer wrapper, other documentary evidence of receipt maintained by the installation, or oral testimony or statements of Government personnel. (4) If an emergency or unanticipated event interrupts normal Government processes so that offers cannot be received at the Government office designated for receipt of offers by the exact time specified in the solicitation, and urgent Government requirements preclude amendment of the solicitation or other notice of an extension of the closing date, the time specified for receipt of offers will be deemed to be extended to the same time of day specified in the solicitation on the first work day on which normal Government processes resume. (5) Offers may be withdrawn by written notice received at any time before the exact time set for receipt of offers. Oral offers in response to oral solicitations may be withdrawn orally. If the solicitation authorizes facsimile offers, offers may be withdrawn via facsimile received at any time before the exact time set for receipt of offers, subject to the conditions specified in the solicitation concerning facsimile offers. An offer may be withdrawn in person by an offeror or its authorized representative if, before the exact time set for receipt of offers, the identity of the person requesting withdrawal is established and the person signs a receipt for the offer. (g) Contract award (not applicable to Invitation for Bids). The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offeror's initial offer should contain the offeror's best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary. The Government may reject any or all offers if such action is in the public interest; accept other than the lowest offer; and waive informalities and minor irregularities in offers received. (h) Multiple awards. The Government may accept any item or group of items of an offer, unless the offeror qualifies the offer by specific limitations. Unless otherwise provided in the Schedule, offers may not be submitted for quantities less than those specified. The Government reserves the right to make an award on any item for a quantity less than the quantity offered, at the unit prices offered, unless the offeror specifies otherwise in the offer. (Applies to offers exceeding $3,000, and offers of $3,000 or less if the solicitation requires the Contractor to be registered in the Central Contractor Registration (CCR) database. The offeror shall enter, in the block with its name and address on the cover page of its offer, the annotation "DUNS" or "DUNS+4" followed by the DUNS or DUNS+4 number that identifies the offeror's name and address. The DUNS+4 is the DUNS number plus a 4-character suffix that may be assigned at the discretion of the offeror to establish additional CCR records for identifying alternative Electronic Funds Transfer (EFT) accounts (see FAR Subpart 32.11) for the same concern. If the offeror does not have a DUNS number, it should contact Dun and Bradstreet directly to obtain one. An offeror within the United States may contact Dun and Bradstreet by calling 1-866-705-5711 or via the Internet at http://fedgov.dnb.com/webform. An offeror located outside the United States must contact the local Dun and Bradstreet office for DUNS number. The offeror should indicate that it is an offeror for a Government contract when contacting the local Dun and Bradstreet office. (k) Central Contractor Registration. Unless exempted by an addendum to this solicitation, by submission of an offer, the offeror acknowledges the requirement that a prospective awardee shall be registered in the CCR database prior to award, during performance and through final payment of any contract resulting from this solicitation. If the Offeror does not become registered in the CCR database in the time prescribed by the Contracting Officer, the Contracting Officer will proceed to award to the next otherwise successful registered Offeror. Offerors may obtain information on registration and annual confirmation requirements via the Internet at http://www.ccr.gov or by calling 1-888-227-2423 or 269-961-5757. (l) Debriefing. If a post-award debriefing is given to requesting offerors, the Government shall disclose the following information, if applicable: (1) The agency's evaluation of the significant weak or deficient factors in the debriefed offeror's offer. (2) The overall evaluated cost or price and technical rating of the successful and debriefed offeror and past performance information on the debriefed offeror. (3) The overall ranking of all offerors, when any ranking was developed by the agency during source selection. (4) A summary of rationale for award; (5) For acquisitions of commercial items, the make and model of the item to be delivered by the successful offeror. (6) Reasonable responses to relevant questions posed by the debriefed offeror as to whether source-selection procedures set forth in the solicitation, applicable regulations, and other applicable authorities were followed by the agency. (End of Provision) ADDENDUM TO FAR 52.212-1(c), Period of Acceptance of Offers. The paragraph is tailored as follows: "The offeror agrees to hold the prices in its offer firm for 60 calendar days from the date specified for receipt of offers." ADDENDUM TO 52.212-1 PROPOSAL PREPARATION INSTRUCTIONS A. To assure timely and equitable evaluation of the proposal, the offeror must follow the instructions contained herein. The proposal must be complete, self sufficient, and respond directly to the requirements of this solicitation. The response shall consist of two (2) separate parts, Part I - Mission/Technical and Past Performance, and Part II - Price Proposal. B. The contracting officer has determined there is a high probability of adequate price competition in this acquisition. Upon examination of the initial offers, the contracting officer will review this determination and if, in the contracting officer's opinion, adequate price competition exists no additional cost information will be requested and certification under FAR 15.406-2 will not be required. However, if at any time during this competition the contracting officer determines that adequate price competition no longer exists offerors may be requested to submit information other than cost or pricing data to support a determination of price reasonableness. C. SPECIFIC INSTRUCTIONS: Proposal Format: Format for proposal - Parts I and II: (a) A page is defined as one face of an 8 ½" x 11" sheet of paper containing information. (b) Typing shall not be less than 12 pitch. (c) Electronic copy shall be submitted in MS Word format or a format that can be read by MS Word. Any spread sheet shall be submitted in Excel format or a format that can be read by Excel. (c) Spiral bound or 3 ring binders, each section shall be tabbed and properly labeled (d) Contractor shall ensure all names of potential employees are removed from proposal Submission requirement- Contractor shall submit two (2) hard copy proposals and one electonic copy. Part 1 and Part II shall be submitted in separate spiral bound or 3 ring binders. Contractor shall ensure both hard copy and electronic proposals are received by 17 Aug 09 at 1400 hours (2:00PM) Central Daylight Savings Time. Hard Copy vs. Electronic Proposals- In the event that there is inconsistency between the hard copy proposal and the electronic proposal, the hard copy proposal shall take precedence. Part I - 1. Mission/Technical Capability and 2. Past Performance Proposal - (Submit as identified in Section C entitled Specific Instructions above). Mission/Technical Capability--No longer than 50 pages, a summary cover page must include contractor POC, address, phone number, DUNS number, CAGE Code, total cost proposed and proposal title. 1. Mission/Technical Proposal shall include offeror's ability to provide the following: - Key personnel proposed for assignment to the project and skill level to include special qualification of key personnel. (Contractor shall ensure all names are removed from proposal) - Persons designated as Project Manager. Submit resumes of personnel; each resume shall be no longer than 3 pages each. (Contractor shall ensure all names are removed from proposal) - Comprehensive technical and management approach to accomplish the work effort. Describe knowledge and understanding of the requirement as outlined in the PWS and Training Dates. The technical approach must identify the methodology and analytical techniques to be used to fulfill the technical requirements. The technical approach should clearly describe the following: - An overview of the proposed methodology in meeting the technical requirements identified in the PWS. - The logical sequence of tasks to be performed in order to accomplish the requirements. - Identify and describe the specific techniques and steps that will be applied during the accomplishment of all task of this project. - Any deliverables to be produced in response to project requirements. 2. Past Performance-No longer than 10 pages. Describe past performance that would qualify your firm for consideration. Describe corporate qualifications and capabilities related to the performance of this PWS requirement. 2. Part II - Price Proposal - Submit separate from Part I (Submit as identified in Section C above entitled Specific Intructions). Price Proposal shall be submitted in MS Excel format or a format that can be read by MS Excel which includes a break out of each of the CLINs, with a subtotal for each CLIN and total proposed cost. All figures shall at a maximum be two places to the right of the decimal point. Provide a detailed cost estimate to include showing all labor categories, hourly rates, G&A, Fringe Rates, Profit, and any other rates that lead to the extended labor rate. Ensure each is subtotaled and totaled for each labor category. Provide additional data to support any anticipated/proposed cost for Travel and Other Direct Cost. Also include a statement in proposal that a Wide Area Work Flow (WAWF) account has been initiated or already established (DoD has mandated the use of WAWF). 52.212-2 EVALUATION--COMMERCIAL ITEMS (JAN 1999) EVALUATION CRITERIA 1. Formal Communication. Proposal Evaluation Factors are as follows: (1) Mission/Technical Capability (a) Sub Factor 1 - Key Personnel (b) Sub Factor 2 - Technical and Management Approach (2) Past Performance (3) Price Factor 1, Mission/Technical Capability. The proposal shall be prepared in an orderly format and in sufficient detail to enable the Government to make a thorough evaluation of the contractor's technical competence and ability to comply with the contract task requirements specified in the PWS. The Government will review applicant qualifications to ensure compliance, as well as other requirements of the PWS. The Mission/Technical Capability evaluation factor will be evaluated on the basis of Personnel Qualifications and Experience and focus on the strengths and deficiencies for Sub factors 1-2 of the offeror's proposal. Sub Factor 1: Key Personnel - Contractor personnel, who read, understand, speak, and write the English language. - Ability to travel to multiple CONUS locations and deliver one-day and two day clinical training workshops. - Ability to travel to multiple OCONUS locations and deliver one-day clinical training workshops. - The trainers must have a Doctoral Degree in Clinical Psychology with appropriate licensure in his/her state. - Contractors resume should exhibit experience in developing research protocols and conducting research in the area of CBT for couples with PTSD or combat related stress reactions. Sub Factor 2: Technical and Management Approach - Must show experience/knowledge of the following: - Demonstrate experience in training in Cognitive Behavioral Treatment (CBT) for couples or conjoint therapy for those with PTSD and other combat stress related reactions. - Demonstrate at least 1 year of personal experience in training others to use the principals of CBT for couples with PTSD and trauma related disorders. - Demonstrate experience in program evaluation including satisfaction outcomes, pre/post knowledge tests for those involved in the training and the tracking patient outcomes. - Keep track of demographic information for all participants and develop a web-based community of practice as described in the PWS. - Provide a CBT Conjoint PTSD therapy training manual that is specifically tailored to military mental health clinicians. Factor 2: Past Performance - Describe past performance that would qualify your firm for consideration. Describe corporate qualifications and capabilities related to the performance of this PWS requirement. Factor 3: Price. The Price Proposal shall include pricing for all Contract Line Item Numbers (CLINs) listed on Page 1 of this solicitation. The Offeror shall identify the proposed Labor Category (ies) and the number of Full Time Equivalents (FTEs) needed to perform each task as part of their price proposal. An assessment of affordability will be performed on the total evaluated price using techniques described in the paragraphs below. The total evaluated price consists of the base period, plus the option periods, and any optional CLINs. Evaluation of options shall not obligate the Government to exercise such options. Reasonableness. The Government will evaluate proposed prices to determine whether prices are realistic for the work to be performed; reflect a clear understanding of the requirements, and are consistent with the various elements of the offeror's mission capability Proposal. Unrealistically low proposed prices, initially or subsequently, may be grounds for eliminating a Proposal from competition either on the basis that the offeror does not understand the requirement or the offeror has provided an unrealistic Proposal. Price analysis techniques may include a comparison of proposed prices received in response to the solicitation, comparison with other contract prices for similar services, and/or a comparison of proposed prices with the independent Government cost estimate. Color or adjectival ratings will not be assigned to the price factor. Completeness. The Price Proposal will be evaluated for completeness in relation to the contractor's proposed technical and management approach, and to ensure it reflects a clear understanding of the requirements.
