- THIS REQUEST FOR INFORMATION (RFI) IS NOT A REQUEST FOR PROPOSAL. It is a market research tool (IAW FAR Part 10) being used to determine potential contractors capable of providing the services described herein prior to determining the method of acquisition and issuance of a Request for Proposal. It is also a tool to get feedback on the requirement detailed in the DRAFT Statement of Work dated July 9, 2009 (attached). Feedback is encouraged and appreciated. A solicitation is not currently available. Responses to this RFI are not considered offers and cannot be accepted by the Government to form a binding contract. Respondents should not submit any proprietary information when responding to this RFI. Respondents should also note that the government will not return any information submitted in response to this RFI. The Government is not responsible for any costs incurred by respondents regarding any responses to this RFI. Respondents must submit capability statements that clearly define the company's ability to perform the work stated in this RFI in conjunction with the DRAFT SOW. PARTICULARS: 1. TITLE: HOME INFUSION SERVICES 2. LOCATION: BAY PINES VA Healthcare System Eligibility Area 3. ANTICIPATED PERIOD OF PERFORMANCE: Up to 5 years (12 month base period and 4 additional 12 month option periods) 4. ANTCIPATED CONTRACT TYPE: TBD 5. DESCRIPTION OF REQUIREMENT: Contractor shall provide all labor, supplies, equipment, medication, infusion, solutions pharmacy compounding (using aseptic techniques in a laminar flow hood) and services necessary to provide home infusion therapy and nursing services to eligible veterans served by Bay Pines VAHCS. Refer to the Attached HOME INFUSION AND NURSING SERVICES "DRAFT" SOW, dated July 9, 2009. 6. SUBMITTAL INFORMATION: It is requested that interested concerns submit a brief capabilities statement package (no more than 10 one-sided pages in length, single spaced, 12 point font minimum) demonstrating their ability to perform the services listed in the attached DRAFT PWS. A generic capability statement is not acceptable. Please review carefully the requirements listed below. This documentation must address, at a minimum, the following: a. Title of the requirement b. Company profile to include number of employees, annual revenue history, office location(s), DUNS number, and a statement regarding current small/large business status and socio-economic classification (i.e. SDVOSB, VOSB, 8(a)/HUBZone). c. Prior/current corporate experience performing efforts of similar size and scope within the last three years, including contract number, organization supported, indication of whether as a prime or subcontractor, contract values, Government point of contact with current telephone number, and a brief description of how the referenced contract relates to the services described herein d. Resources available such as corporate management and currently employed personnel to be assigned to tasks under this effort to include professional qualifications, specific experience of personnel. e. Management approach to staffing this effort with qualified personnel f. Statement regarding capability to meet requirements discussed in the DRAFT SOW g. Company's ability to begin performance upon contract award h. Identify if the company currently provides the same service to commercial (non-government) entities and the customary contract type, price discounts and billing practices that are employed. i. List of anticipated HCPCS/CPT codes that are applicable for the services and equipment/materials that would typically be provided by a company performing the services described in the "DRAFT" Statement of Work (This list is not included in the 10 page limitation) j. If HCPCS/CPT codes are utilized, how would the company recommend resolving potential coding errors, coding disputes and/or reimbursement for services/equipment/materials that may be required, but not specifically covered by the contract? k. Any recommended changes and/or comments to the "DRAFT" SOW (This information is not included in the 10 page limitation).
