- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a separate written solicitation will not be issued. This solicitation number is NHLBI-PB-(HG)-2009-245-GMV and is issued as a Request for Quotation (RFQ). The solicitation/contract will include all applicable provisions and clauses in effect through Federal Acquisition Circular 2005-34 (7/1/2009). The North American Industry Classification (NAICS) Code is 423450 and the business size standard is 100 employees. This acquisition is being conducted using Simplified Acquisition Procedures in accordance with FAR Part 13. However, this solicitation is not set aside for small business. It is the intent of the National Institutes of Health (NIH) National Heart, Lung and Blood Institute to negotiate and award a purchase order, on a sole-source, non-competitive basis with Affymetrix, Inc. 3420 Central Expressway, Santa Clara, CA 95051 to purchase Affymetrix items listed below: 1.Item Number: 901153, Array, Genome-Wide Human SNP 6.0, 50 pack One (1) Box 2.Item Number: 901152 Kit, Genome-wide Human SNP NSP/STY Assay 5.0/6.0 50RXNOne (1) Kit 3.Item Number: 901159, Kit, Genome-wide Human SNP NSP/STY Assay Kit 5.0/6.0 48 RXOne (1) Kit 4.Item Number: 901012, Genomic DNA StandardOne (1) Each 5.Item Number: 901015, Kit, Genome-wide Human SNP NSP/STY 5.0/6/0 100RXNOne (1) Kit 6.Item Number: 901011, Kit, Genome-wide Human SNP/STY Assay 5.0, 100RXNOne (1) Kit 7.Item Number 901012: : Genomic DNA StandardOne (1) Each 8. Item Number 901150, Array, Genome-wide Human SNP Array 6.0 100 packOne (1) Box Delivery will be on or before Two (2) weeks after receipt of order (A.R.O.) This item is the human single nucleotide polymorphism (SNP) mapping chip and its assay kit from Affymetrix, Inc. The human genome-wide SNP array 6.0 has 906,600 SNPs and 946,000 copy number variation probes on each chip from various genes in human chromosome. Aside from its ability to produce vast amount of genotype data in such a short period of time, NHGRI will utilize this item to find candidate genes responsible for aggressive prostate cancer by associating SNPs and copy number polymorphisms discovered or analyzed through whole-genome wide association studies by running this chip on 150 affected and unaffected individuals. Affymetrix’s genome-wide human SNP array 6.0 set has higher number of SNPs on each chip allowing for very dense scan of the genome which in turn will increase the amount of information that can be extracted for association studies compared to other available chips. It provides robust, flexible, cost-effective approach for obtaining large amount of genotype data. In addition, NHGRI has a microarray core facility where they have all the necessary hardware equipments and many reagents in stock to run this assay. They have optimized the protocol and perfected its method for running this assay. The core provides service to NHGRI to run chips after labeling process at no cost. Therefore, NHGRI will only have to purchase chips and labeling kit to carry out this project and use the core for the rest of the process The sole source determination is based on the fact that Affymetrix is the only known source capable of providing the required supplies, and are the only items that are compatible with government-owned Affymetrix equipment. The Affymetrix Genome-wide SNP chip carries a unique set of SNPs compared to other platforms on the market. In order to merge data sets, as well as retain maximum number of SNPs from previously generated dataset, NHGRI will need to continue to use the platform throughout the prostate cancer association study. A change in the product would affect ongoing laboratory research and threaten the investment already made. The solicitation incorporates the provisions at FAR 52.212-1, INSTRUCTIONS TO OFFERORS - COMMERCIAL ITEMS - DEVIATION FOR SIMPLIFIED ACQUISITIONS, and at FAR 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS - WITH DUNS NUMBER ADDENDUM. The resulting contract will incorporate the requirements of the clause at FAR 52.212-4, CONTRACT TERMS AND CONDITIONS - COMMERCIAL ITEMS, and the requirements contained in paragraphs (a) and (d) of the clause at FAR 52.212-5, CONTRACT TERMS AND CONDITIONS REQUIRED TO IMPLEMENT STATUTES OR EXECUTIVE ORDERS - COMMERCIAL ITEMS - DEVIATION FOR SIMPLIFIED ACQUISITIONS. The contract will also incorporate the following FAR clauses cited in paragraphs (b) and (c) of the latter clause: FAR 52.222-26, EQUAL OPPORTUNITY; FAR 52.222-35, AFFIRMATIVE ACTION FOR SPECIAL DISABLED AND VIETNAM ERA VETERANS; FAR 52.222-36, AFFIRMATIVE ACTION FOR HANDICAPPED WORKERS; FAR 52.222-37; EMPLOYMENT REPORTS ON SPECIAL DISABLED VETERANS OF VIETNAM ERA; and FAR 52.225-3, BUY AMERICAN ACT - SUPPLIES. This action is under the authority of 41 U.S.C. 253(c) (1), as set forth in FAR 6.302-1 and HHSAR 306-302-1. Only one responsible source and no other supplies or services will satisfy agency requirement. Interested parties may identify their interest and capability to respond to the requirement or submit proposals. This notice of intent is not a request for competitive quotations however; all responses received within eight (8) days from the date of publication of this synopsis will be considered by the Government. A determination by the Government not to compete this proposed acquisition is based upon responses to this notice and is solely for the purpose of determining whether to conduct a competitive acquisition. The offeror must include in their quotation, the unit price, the list price, shipping and handling costs, the delivery period after contract award, the prompt payment discount terms, the F.O.B. Point (Destination or Origin), the Dun & Bradstreet Number (DUNS), the Taxpayer Identification Number (TIN), and the certification of business size. Note: In order to receive an award from the NHLBI contractors must have a valid registration in the Central Contractor Registration (CCR) www.ccr.gov. Interested vendors capable of furnishing the government with the item specified in this synopsis should submit their quotation to the below address. Quotations will be due eight (8) calendar days from the publication date of this synopsis or by August 5, 2009, 3:00 p.m. Eastern Standard Time.
