Description
FILL IN PROVISION AND RETURN WITH QUOTE. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4539155A004; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-34, effective date July 01, 2009; Defense DCN 20090115, effective date 15 Jan 2009; and AFAC 2009-0318 effective date 18 Mar 2009. The North American Industry Classification System (NAICS) code is 811219. The business size standard is $7,000,000.00. The Federal Supply Class (FSC) is J099. The Standard Industrial Classification (SIC) is 7699. Travis AFB intends to award a Sole Source requirement for onsite inspection and repair of various instruments (a list can be provided upon request) to include training on findings and a final presentation of findings as well as a report in writing. This work is to be completed with the final presentation taking place no later than 27 Aug 09. CLIN0001- Non-personal Service: Contractor shall provide onsite inspection and repair of various instruments located in the following departments of David Grant Medical Center (DGMC): OBGYN, General Surgery, Orthopedics, and Neurosergery. Inclusive of this service, contractor shall evaluate the Central Processing and Distribution processes of decontamination, assembly, maintenance, usage and storage. Training and a final presentation including a written report will be provided based on findings. All labor, travel, equipment, parts, training, and consultation shall be included in this not to exceed (NTE) estimate. Spectrum Surgical Instruments Corp. shall provide: preventative maintenance, instrument repair, inspection, replacement and or cleaning for all types of surgical instrumentattion regardless of manufacturer; customer service/tech support 24 hours/7 days a week; comprehensive educational programs on proper inspection, care and handling and critical factors pertinent to patient safety; a daily invoice annotating instrument trays/departements that were serviced; Spectra-Scan digital mapping technology; cleanup of any and all areas where maintenance has been performed. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items (June 2008). FAR 52.212-2 Evaluation -- Commercial Items (Jan 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: 1: Price, 2: Technical acceptability of the service offered (must cover all facets of the service outlined above; company must be American Society for Quality (ASQ) and International Standards Organization (ISO) certified; must be able to complete service and provide final report NLT 27 Aug 09 ).
