Attachment 4: North River Road map Attachment 3: Bald River Road map Attachment 2: Area Map Attachment 1:Schedule of Items and Table 1: Aggregate Percentages Cherokee National Forest, South Zone Engineering has a requirement for furnished and tailgate spread or stockpiled crushed aggregate on North River (#217) and Bald River (#126) Roads on the Tellico Ranger District, Cherokee National Forest, in Monroe County, TN. Gradations required are: grade 57, grade 4, crusher run, and surge riprap. PROJECT TITLE: Furnish Aggregate, Cherokee National Forest, South Zone Engineering. This solicitation identified as AG-4756-S-09-0047 is issued as a Request for Quotation using simplified acquisition procedures. Issue date of this synopsis/solicitation is July 30, 2009. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6 of the Federal Acquisition Regulations, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. SET-ASIDE: This solicitation is a Total Small-Business Set-Aside. The applicable NAICS code is 484220; the applicable small business size standard is $23.5 million dollars average annual receipts for the preceding three fiscal years. PROJECT LOCATION: North River Road: From Tellico Plains, TN at the intersection of TN Hwy 68 and TN Hwy 165, travel approx. 5 miles on TN 165 to a right turn on FS Road #210 (Tellico River Road). Travel approx. 17 miles on FS Road #210 to FS Road #217 (North River Road). Turn left onto FS Road #217. Project begins approx. 0.6 miles from FS Rd. #210 and continues for 10 miles. Bald River Road: From the intersection of FS Road #210 and #217, travel approx. 4 miles to FS Road #126 (Bald River Road) on right. Turn right onto FS Road #126. Project begins approx. 3 miles from FS Road #210 and continues for 6 miles. DESCRIPTION OF REQUIREMENT: Contractor shall furnish all materials, transportation, supervision, labor, and equipment with operators necessary to furnish, tailgate spread or stockpile aggregate surfacing at specified locations in strict accord with specifications, terms, and conditions herein. Aggregate shall be spread uniformly across the width of the roadbed. The period of this contract shall be 120 days from date of award. The anticipated start date is August 19, 2009. Award will be made on a best-value basis to the contractor offering the highest total tonnage of aggregate. The application rate shall vary according to the specific road segment as directed by the Contracting Officer’s Representative (COR) or Inspector. Delivery shall be determined at the time of issuance of the Notice to Proceed and will be dependent on weather and coordination with other Forest Service contracts. Placement shall be as directed by the Forest Service on-site representative. Delivery times shall be scheduled so as to arrive at placement locations between the hours of 8 a.m. and 5 p.m. Monday through Friday unless prior arrangements have been agreed upon. There are likely to be individual days or a series of days when delivery is not needed during the contract period. The aggregate provided shall be one or any of the TDOT gradations referenced in Table 1. The exact quantity of each gradation is unknown, but the estimated percentages for each gradation to be delivered are indicated in Table 1, Percentages, as shown on Attachment 1. The CO will indicate to the contractor the quantity and gradation needed on daily basis during the contract period. The materials and their use shall comply with TNDOT specifications for highway use. The aggregate sources shall be approved by the Forest Service before any materials are delivered to the site. Weight tickets from sources shall constitute a certificate of compliance for quality requirements. The weight of the aggregate shall be reduced by moisture content in excess 7%. Trucks used for hauling the aggregate material shall have a clean, tight bed and have a minimum payload capacity of 20 tons. The number of trucks required per day will vary from one to three. Aggregate shall be spread or stockpiled as directed by the CO. Inspection of facilities and equipment and sampling and testing of materials shall be allowed at all segments of the total process. Acceptance of the in-place aggregate shall be contingent upon approval by the Forest Service. Materials and in-place product shall comply with industry standards. Certified weight tickets for the aggregate delivered and placed will be the basis for payment. SUBMIT QUOTES: On company letterhead or form, using attached Schedule of Items, no later than close of business August 13, 2009, by regular or express mail, to: Cherokee National Forest, Attn: Nina Barrow, 2800 North Ocoee Street, Cleveland TN 37312. Faxed quotes are not permitted. TIEBREAKER PROCESS: In the event of a tie in total tonnage offered, the following order of priority will be followed: 1.HUBZone certified concerns 2.Service Disabled Veteran Owned concerns 3.8(a) firms 4.Public Drawing of lots Award will be made on a best value basis. Quotes will be evaluated on price, past performance and experience; submit 3 projects of a similar nature with references for each project. A fixed price contract will be awarded from this solicitation. SPECIAL NOTE: Successful contractor must be registered in the Central Contractor Registration Database (www.ccr;gov) and in Online Representations and Certifications (http://orca.bpn.gov) prior to award of contract. Vendors must submit a DUNS number with their quotation.
