Description
Supply List The Centers for Disease Control and Prevention (CDC) intend to purchase laboratory reagents and instruments, from Applied Biosystems, LLC, 301 Merritt 7, Norwalk, CT 06851-1070, under a Firm Fixed Price Purchase Order. The Hereditary Blood Disorder Branch of the Centers for Disease Control and Prevention conducts ongoing blood components that are needed by hepatitis antigens and antibodies and are a crucial components of monitoring the blood supply. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice.The Government intends to evaluate offers and award a contract without discussion to one Offeror capable of providing the exact product listed. The initial offer should contain the offeror's best terms from a technical and price standpoint. The Government anticipates awarding a contract to the offeror whose proposal is the most advantageous to the Government (lowest price). Offerors' price proposal will be evaluated to determine price reasonableness and delivery terms. Prices that are excessively high or low may be considered unrealistic, unreasonable, and may receive no further consideration. The North American Industry Classification (NAICS) Code is 334516, and the Small Business Size Standard is 750 Employees for this requirement. Quote shall include all fees, including shipping or freight costs; provide exact delivery schedule to include the number of days of the anticipated delivery. CDC expects awardee to deliver within 2 weeks of award of the purchase order. NO partial shipments unless specifically authorized at time of award. Upon delivery, all shipping documents and containers will include CDC order number. Warranty and all required Terms and Conditions of product merchantability must be included with contractor's quote. Quote MUST be good for 30 calendar days after submission. Invoices will be submitted in accordance with the resulting instructions included in the Purchase Order at award. The selected Offeror must comply with the following commercial item terms and conditions. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. 52.212-2 Evaluation Commercial Items (JAN 1999); the evaluation criteria stated in paragraph (a) of this provision are as follows: (1) technical capability of the proposed supplies to meet the requirements; and (2) price. OFFERORS SHALL INCLUDE A COMPLETED COPY OF THE PROVISION AT 52.212-3, OFFERORS REPRESENTATIONS AND CERTIFICATIONS - COMMERCIAL ITEMS, WITH THEIR QUOTE. To complete this certification download the FAR Clauses (Volume II) at: http/www.arnet.gov/far, print out the clause 52.212-3, Offeror Representation and Certification, and fill in the appropriate blocks. FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. This requirement will be awarded based upon lowest price technically acceptable. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is RFQ # 2009-Q-11583, and is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-31. The following FAR Clauses are hereby added: 52.247-35 F.O.B. Destination; The clause at 52.212-5 Contract Terms and Conditions required to Implement Statutes or Executive Orders - Commercial Items applies to this acquisition. The following clauses listed in paragraph (b) of 52.212-5 apply to this acquisition: (1), (3), (5), (7), (9), (11), (12), (14), (15), (17), (18), (20), and (24) Terms and Conditions. Simplified Acquisitions (Other than Commercial Items) (MAR 2009) The contractor shall comply with the following Federal Acquisition Regulation (FAR) clauses incorporated by reference: FAR 52.252-2 Clauses incorporated by Reference (Feb 1998) This contract incorporates one or more clauses by reference, with the same force and effect as if they were given in full text. Upon request, the Contracting Officer will make the full text available. Award cannot be made to contractors not registered and active in CCR. If not registered, contractors may access the Central Contractor Registry (CCR) Web site at www.ccr.gov. Quote must include proof of CCR registration. This procurement is not set aside for small business. Quotes will be due within 10 days of this synopsis or by 5:00p.m., Eastern Standard Time (EST) on August 10, 2009 at the Centers for Disease Control and Prevention, Acquisition & Assistance, Branch 3, Team I, Attn: Maria Nichols, 2920 Brandywine Road, Room 3727.01, Atlanta, GA 30341-3724.
