- Please note that this is NOT a request for quotes or proposals, the Government is seeking information for market research purposes only. The Government may or may not issue solicitation documents. The Richard L. Roudebush VA Medical Center (RLR VAMC), Indianapolis, IN, is conducting a market survey for qualified firms capable of providing the required bed services outlined below. Applicable NAICS Code is 811219, size standard $7.0M. Responses to this notice MUST be submitted in writing (email or fax is acceptable) and received no later than August 05, 2009. No telephone inquiries will be accepted. Description of work to be performed includes: Contractor shall provide all supervision, labor, tools, parts, materials and travel required to provide PREVENTATIVE/ELECTRICAL SAFETY AND EMERGENCY maintenance for government-owned Hill-Rom hospital beds at the RLR VAMC, 1481 W. 10th Street, Indianapolis, IN 46202. One (1) annual preventative maintenance (PM) inspection shall be made per bed per year with no time lapse greater than 13 months. Contractor shall be able to provide maintenance and repairs based on availability of beds. The PM shall correct any equipment malfunctions and assure that the equipment meets the manufacturer's maintenance specifications and factory tolerances. During the PM visit, the Contractor shall replace all worn parts as necessary for the satisfactory function of the equipment (only OEM parts shall be used). Includes emergency service calls during the hours of Monday through Friday from 8am to 4:30pm. Contractor shall furnish and install all parts necessary to repair the equipment. The Contractor shall submit, with offer, written procedures for the VA to contact the Contractor when emergency service is needed. Procedure information shall include service representative's name, address and phone number. An alternative phone number shall also be provided. The telephone service technician shall return a VA request for emergency service by phone within two (2) hours. If the problem cannot be solved over the telephone within the additional two (2) hours, the call shall be referred to the Contractor to the service representative. The service representative shall appear on site within 24 hours or a time agreeable with the COTR. Only the COTR or designee can determine when the equipment is properly repaired and operating within the manufacturer's tolerance. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, HubZone, 8(a), small disadvantaged business, small business or large business) relative to NAICS 811219 (size standard of $7.0M). Responses to this synopsis will be sued by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, a service capability statement, examples of similiar facilities to which you have provided similiar services to, DUNS Number, address, point of contact and the size of your business pursuant to the following questions: (1) Is your business large or small? (2) If small, does your firm qualify as a small, emerging business or small disadvantaged business? (3) If disadvataged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified HubZone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business?
