- This is a combined synopsis/solicitation prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-35. The following provisions and clauses apply plus any addenda: FAR 52.212-1 Instructions to Offerors - Commercial Items; 52.212-2 Evaluation - Commercial Items; 52.212-3 Offeror Representations and Certifications --Commercial Items; 52.212-4 Contract Terms and Conditions - Commercial Items; and 52.212-5 Contract terms and Conditions to Implement Statutes or Executive Orders - Commercial Items. NAICS Code for this requirement is 339112. The U.S. Naval Hospital, Okinawa, Japan intends to negotiate a sole-source contract with Terumo Medical Corporation for Quadruple Blood Bags. However, any firm that believes it can meet the requirements may give written notification to the Point of Contact (POC). Supporting documents must be furnished in sufficient detail, to demonstrate the contractor’s capability to provide and ability to comply with the requirements. Contractor must be registered in Central Contractor Registration. [Note: Lack of registration in the Central Contractor Registration will make a Contractor ineligible for award] Registration is available via the Internet at www.ccr.gov. Requirements are to be delivered beginning Oct 1, 2009 through September 30, 2010 and include the following schedule of supplies: CLIN 0001: CPD/OPTISOL QUADRUPLE BLOOD BAG WITH 16GA ULTRA THINWALL DONOR NEEDLE, 450ML XT PRIMARY CONTAINER WITH ONE 500ML XT612 PLUS 5-DAY PLATELET BAG AND TWO 400ML SATELLITE BAGS: P/N 1BB*AGQ456A2; 18EA/CASE FOR TOTAL OF 336. 28 CASES TO BE DELIVERED MONTHLY. UNIT PRICE _________________, TOTAL PRICE___________________. CLIN 0002: GREEN TUBING CLIPS. P/N:1BB CLIPS. ONE CASE PER SHIPMENT (MONTH). PRICE _________________, TOTAL PRICE___________________.The contractor shall be responsible for delivery to Tracy CA, 95304.The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: (1) Technical capability of the item offered to meet the Government requirement (including submission of descriptive literature); (2) Price. Quotes must be submitted to the POC no later than Aug 25, 2009; 4:00 PM; Japan Standard Time. Quotes shall be submitted via e-mail in Word, Excel, or PDF format to the POC, Michiko Ura, at michiko.ura@med.navy.mil faxed to 011-81-989-92-8575. Responses received will be evaluated, however, a determination by the Government not to compete the proposed procurement based upon responses to this notice is solely within the discretion of the Government. If no responses are received, U.S. Naval Hospital, Okinawa, Japan will proceed with the sole source procurement.
