- Notice of Intent to Sole Source: VR-12 Health Survey Date: August 10, 2009 Intended Source: Center for the Assessment of PharmaceuticalPractices (CAPP)Boston University School of Public Health715 Albany Street, Talbot BuildingBoston, MA 02118Phone: (617) 638-4640 The Department of Veterans Affairs (VA) Care Coordination Services (CCS)uses the Veterans RAND 12 Item Health Survey (VR-12), a veteran patientspecific version of the SF-12, to collect health status information fromCare Coordination Home Telehealth (CCHT) patients and automate theprocess of data collection. VA requires Contractor support to assessthe Veterans RAND 12 Item Health Survey (VR-12) administration and datacollection methods used in the CCHT program. The Government intends to solicit and negotiate with a single sourceunder the authority of FAR 13.106-1(b)(1). The contract will have athree month base period and a twelve month option period. Interestedparties capable of responding to the requirement may submit a capabilitystatement specifically identifying their ability to accomplish thisrequirement. Responses are due within 10 days after publication of thisnotice. This is not a request for proposals. Responses shall bereviewed solely for the purpose of determining whether to conduct acompetitive procurement. A determination by the Government not tocompete this proposed acquisition following consideration of responsesto this notice is solely with the discretion of the Government.
