Description
Perform removal of all soil, vegetation and trees from traffic islands in the parking lot in front of building 2700. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation number W91248-09-B-A002 is issued as an Invitation for Bid (IFB). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-35. NAICS is 561730; small business size standard is $7M; acquisition is set-aside for HUBZone small business concerns. Offer schedule with Contract Line Item Numbers (CLIN) Items, quantities and unit measure, Scope of Work (SOW,) Drawings, and Wage Determination are available at the Fort Campbell Directorate of Contracting web site: https://doc.campbell.army.mil/opportunities.asps. Performance period is 30 days, which includes (3) three adverse weather days. The Governments requirement is prescribed in the attached Statement of Work. *** A Site Visit will be conducted on 18 Aug 2009 at 1000 hours, all interested parties will meet at building 2172, address is: 2172 13 Street, Fort Campbell, KY. Visitors to Fort Campbell must enter through Gate 4 to obtain a visitor pass. *** The provision at 52.212-1, Instruction to Offerors Commercial Items, is applicable and is tailored as follows: Paragraphs (b) 4, (b) (5), (d), (e), (g), (h) and (i) are deleted. This provision is also addended to incorporate 52.252-1. Solicitation Provisions Incorporated by Reference (Fill-in information www.gsa.gov ); 52.204-6 Data Universal Numbering System (DUNS) Numbers; 52.204-7 Central Contractor Registration; 52.216-1 (fill-in information, firm fixed price); 252.204-7001 Commercial and Government Entity (CAGE) Code Reporting. The Government intends to make a single award to the responsive and responsible bidder submitting the lowest priced offer. The successful bidder must be registered in Central Contractor Registration (CCR.) The CCR website is: https://www.ccr.dlis.dla.mil/ccr/scripts/index.html. Bidders are instructed to submit the following documents with offers: Completed bid schedule, Completed copy of FAR Provision 52.212-3, Offeror Representations and Certifications Commercial Items, and DFARS Provision 252.212-7000 Offeror Representations and Certifications Commercial Items or a statement that Representations and Certifications are available at the ORCA website. Financial and customer references (Name of financial institution where business bank account is located supplier names where accounts are established, account numbers, names of customers, points of contact, and telephone numbers.) Also provide brief description of previous contracts: Services provided the length of the contract agreement, and the dollar value, contact and telephone number. This information will be used to determine responsibility of the successful offeror. The clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition, and is addended to incorporate the following clauses: 52.228-5 Insurance Work on a Government Installation; 252.223-7006 Prohibition on Storage and Disposal of Toxic and Hazardous Materials; 5152.209-4000 DoD Level Antiterrorism (AT) Standards. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable to this acquisition. The following clauses are applicable: 52.203-6, Restrictions on Subcontractor Sales to the Government, with Alt I; 52.219-3 Notice of Total HUBZone Set-Aside; 52.222-3 Convict Labor; 52.222-19 Child Labor Cooperation with Authorities and Remedies; 52.222-21 Prohibition of Segregated facilities; 52.222-26 Equal Opportunity; 52.222-39 Notification of Employee Rights Concerning Payment of Union Dues or Fees; 52.222-41 Service Contract Act of 1965; 52.222-42 Statement of Equivalent Rates for Federal Hires; 52.222-44 Fair Labor Standards Act and Service Contract Act; 52.222-50 Combating Trafficking in Persons; 52.232-33 Payment by Electronic Funds Transfer Central Contractor Registration. DFARS 252.212-7001 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Applicable to Defense Acquisitions of Commercial Items is applicable and the following additional DFARS clauses cited within the clause also applicable: 52.203-3 Gratuities; 252.232-7003 Electronic Submission of Payment Requests and Receiving Reports; 252.247-7023 Transportation of Supplies by Sea.
