Description
Responses to Additional Questions. Also, there will be no more questions allowed after 4:00 p.m. EST on 13 August 2009. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. This solicitation is issued as Request for Quotation (RFQ) No. FA4416-09-T-0030. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular (FAC) 2005-33. The North American Industry Classification System Code (NAICS) for this acquisition is 493190 with a size standard of $25.5 million. The proposed contract is 100 percent set aside for small business. CONTRACT LINE ITEM NUMBERS: 0001: BASE YEAR: Record storage, removal, retrieval and access to medical records. QTY: 12 MONTHS, UNIT PRICE: $_____________, EXTENDED PRICE: $________________. Period of Performance for Base Year (ESTIMATED): 10 August 2009 through 09 August 2010. 1001: OPTION YEAR I: Record storage, removal, retrieval, and access to medical records. QTY: 12 MONTHS, UNIT PRICE: $_____________, EXTENDED PRICE: $________________. Period of Performance for Option Year I (ESTIMATED): 10 August 2010 - 09 August 2011. 1. SCOPE OF CONTRACT 1.1. The contractor shall provide staging, removal, and storage for all records in building 1054. 1.2. The contractor shall bear all costs including labor, parts, tools, travel and such other expenses as are necessary to keep records in good condition, and prepared to provide delivery of records within 30 - 45 minutes of request. 1.3. The contractor shall provide indexing of all records for easy access and inventory purposes. 1.4. The hours of operations are Monday - Friday, excluding holidays 0800 - 1600. 1.5. The contractor must meet HIPAA requirements for handling patient medical information. 1.6. The contractor must prepare records for all National Archives and Records Administration audits and inspections. 1.7. The contractor must store records within a 45 minute drive from Andrews AFB, Maryland. 2. PLACE OF PERFORMANCE 2.1. Malcolm Grow Medical Center, Building 1054, Medical Warehouse for initial removal. 2.2. Subsequent actions will be at the contractor's storage facility. The contracting officer shall follow FAR 52.212.4, Contract Terms and Conditions-Commercial Items for contractor's failure to perform satisfactory services or failure to correct non-conforming services. FAR PROVISIONS: The provisions at 52.212-1, Instructions to Offerors-Commercial Items and Addendum to FAR CLAUSE 52.212-1 applies to this acquisition are as follows: 52.252-1 http://farsite.hill.af.mil, 52.252-5: (b) DFARS (Chapter 2), and 252.225-7000. FAR 52.212-2, Evaluations-Commercial Items, applies to this acquisition. EVALUATION PROCEDURES: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer, conforms to the solicitation and price is deemed most advantageous to the Government in accordance with the requirement. Only those offerors determined to be in compliance with the requirements as stated in this solicitation will be considered for award. If additional information is required to support price reasonableness, each offeror may be required to provide a hard copy of their price break-down. Non-conformance with this requirement may result in the offerors quote being determined unacceptable. FAR 52.212-3, "Offeror Representatives and Certifications-Commercial Items" applies to this acquisition and offeror(s) must include, a completed copy with its offer.
