- Statement of Work Request for Quote (RFQ) The Department of Health and Human Services is currently seeking potential Quotes for Video Teleconference Public Rental Services. This is a combined/synopsis for commercial items prepared in accordance with the format of FAR subpart 12.6, in conjunction with FAR subpart 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation, quotes are being requested and a written solicitation will not be issued. The RFQ number is RFQ# OS995 is hereby issued as a (RFQ), unless otherwise indicated herein. The required period of performance is for one year from the date of award with four 12 months option years. The award will be made as a firm fixed-price contract. The government intends on awarding multiple indefinite delivery indefinite quantity contract. The contractor shall perform work in accordance with the attached Statement of Work. The selected vendors shall provide an established network of at least 100 facilities that is substantially national in scope including Alaska, Hawaii, and Puerto Rico. All responses to the RFQ should be submitted via e-mail to Kenneth.Gipson@psc.hhs.gov. In the written request please reference RFQ # OS995. This is an unrestricted buy. The North America Industry Classification System (NAICS) is 517910. Award will be made to the lowest price technically-acceptable offer. The solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Regulation. The provision at 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. The provision at 52.212-2, Evaluation-Commercial Items, applies to this acquisition. Offerors must include a completed copy of the provision 52.212-3, Offeror Representations and Certifications-Commercial Items, with the offer. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes of Executive Orders-Commercial Items, applies to this acquisition.
