Description
Protestant Evangelical Musician - Statement of Work This is a combined solicitation/synopsis for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is issued as a Request For Quotation (RFQ), solicitation number FA4608-09-Q-S250, in accordance with FAR Parts 12 and 13. The North American Industry Classification System (NAICS) number is 813110, and the business size is $7,000,000.00. The proposed acquisition is set-aside for small businesses. Small businesses, woman-owned, and disabled veteran-owned businesses are encouraged to participate by submitting quotes. The service being procured is as follows: Protestant Evangelical Musician - See Attached Statement of Work Offeror Must submit Quote, Resume with references and Set-Up an appointment for an Audition with a committee - Point of Contact : TSgt Steven Beasley, 318-456-2111 CLIN 0001: Protestant Evangelical Musician Period of Performance 1 Oct 09 through 30 Sep 10 Price Per Unit: _______________ x 104 Units = ____________________ CLIN 1001: Protestant Evangelical Musician Period of Performance 1 Oct 10 through 30 Sep 11 Price Per Unit: _______________ x 104 Units = ____________________ Offerors must comply with all instructions contained in FAR 52.212-1, instructions to offerors Commercial Items, applies to this acquisition and the following addenda applies. Offerors shall submit their resume and quote on letterhead stationery annotating Cage Code Number, DUNS Number and Tax Identification Number. THE GOVERNMENT WILL NOT ACCEPT INCOMPLETE PROPOSALS. Offers will be evaluated on technically acceptable, price and in compliance with all directions in this solicitation and attached statement of work. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. If an offer cannot comply with every requirement, that offer will not be technically acceptable, and therefore will not be considered. Before an award can be made, according to DFARS 252.204-7004 Alt A, offeror must be in the Contractor Central Registration (www.ccr.gov) database and also in the Online Representation and Certifications database (https://orca.bpn.gov/).
