- This is a Pre-Solicitation notice. U S Naval Hospital, Guam is soliciting for a firm fixed price type contract for Renovation/Upgrade of Waiting Rooms: C-400 General Surgery, E-200 Eye Clinic, F-204 Multi-Service Unit and J-201 ENT Clinic. The U S Naval Hospital, Guam is seeking for a contractor to provide all labor, materials, tools, equipment and supervision for the proposed contract. The solicitation and any subsequent will be posted on the Navay Electroni Online (NECO) website: www.neco.navy.mil. The contract is procured utilizing Simplified Acquisition Procedures. Awardee is mandatory to have a Cage Code and DUN # and must be registered on Central Contracting Registration (CCR) www.ccr.gov and Wide Area Work Flow (WAWF) https://wawf.eb.mil Area of consideration is Guam widearea ONLY. PLEASE SEE STATEMENT OF WORK (SOW) BELOW FOR DETAILS: U. S. NAVAL HOSPITAL, GUAM FACILITIES MANAGEMENT DEPARTMENT ISTATEMENT OF WORK: The contractor shall provide all labor, materials, equipment, design built if required, supervision and incidental related work necessary for upgrading of C400, E200 and J201 Waiting Rooms. The work includes, but is not limited to the following: A. C-400, GENERAL SURGERY WAITING ROOM 1) Conceal exposed electrical conduits and wires to include relocation of TV outlet. 2) Replace existing chair rails to match chair rails at ICU Waiting Rooms. 3) Perform exploratory inspection of wooden junction box at south wall, if box is still active replace box with new to modernize and suit new condition. 4) Fabricate and erect approx. 4'W x 8'H fire-rated gypsum wall at south wall behind existing entrance RH door, remove existing magnetic holder w/bracket and install flush mounted to new wall. 5) Replace temporary partition adjacent to the Check-in Office with fire rated gypsum board wall approx. 6’-0"W x 6’-0"H. 6) Replace existing 4" cove base thru-out entire new Waiting Room. 7) Replace dirty and soiled ceiling tiles. 8) Prepare, prime and paint walls two (2) tone color to match walls at ICU Waiting Rooms. B. E-200, EYE CLINIC WAITING ROOM 1) Conceal exposed electrical conduits and wires in Waiting Room Area to include relocation of 4 each duplex outlets to the standard height. 2) Relocate one TV outlet and one Hospital TV monitor. New location shall be behind the two units. 3) Remove and seal up all existing nurse call console. 4) Fabricate and Install 1” x 3” chair rails in the Waiting Area to match the ICU Waiting Room chair rails. 5) Replace dirty and soiled 2x4 ceiling tiles. 6) Prepare, prime and paint (2 TONE) corridor walls, extending from E200 Main entrance to E200A to include refurbishing of existing wooden handrails. NOTE: Colors to match walls at the ICU Waiting Room. STATEMENT OF WORK for Waiting Rooms in C400, E200, F204, & J201Page 2 7) E200A WAITING ROOM AND CHECK-IN AREAS: Prepare, prime and paint (2 TONE) walls complete. NOTE: Colors to match walls at the ICU Waiting Room. 8) Replace/Install new 4" cove base to match new paint. C. F-200, MULTI-SERVICE UNIT WAITING ROOM 1) Replace existing vinyl floor tiles with 12” x 12” ceramic tiles to match floor tiles in ICU Waiting Rooms. 2) Replace existing vinyl cove base with 4” x 12” ceramic tiles matching the floor tiles. 3) Provide and install 1" x 3" chair rail similar to E300 waiting room. Conceal exposed electrical conduits/wires and replace boxes and covers. 4) Replace soiled and dirty ceiling tiles to include cleaning and painting of AC diffusers. 5) Patch cracks and holes on walls. 6) Prepare, prime and paint walls two (2) tone color to match F3 MBU Waiting room. 7) Clean up jobsite, dispose of debris and demobilize. D. J-201, ENT CLINIC WAITING ROOM/FRONT DESK/RESTROOM 1) Conceal exposed electrical/telephone conduits and wires in Waiting Room Area to include relocation of 4 each duplex outlets to the standard height. 2) Replace existing chair rails to match chair rails in ICU Waiting Rooms. 3) Replace carpet with 12” x 12” ceramic tiles to match floor tiles in ICU Waiting Rooms. 4) Replace vinyl cove base with 4” x 12” ceramic tiles matching the floor tiles. 5) Prepare, prime and paint (2 tone color) walls of J201 waiting room and front check-in desk area NOTE: Colors to match walls at the ICU Waiting Room. 6) Replace both Restroom and Storage doors complete and paint to match new walls. 7) Replace dirty and soiled ceiling tiles throughout area. 8) CO-ED RESTROOM: a. Replace both floor and wall tiles to match ICU Restroom floor and wall tiles. b. Paint walls above wall tiles to include ceiling in the Restroom. c. Replace the following: mirror, stain steel metal shelf, lavatory and faucet, paper towel holder, toilet paper holder, and commode (HC Type). 9) SUPPLY CLOSET: a. Remove existing wooden panel. b. Prepare, prime and paint walls complete. STATEMENT OF WORK for Waiting Rooms in C400, E200, F204, & J201Page 3 IISPECIAL CONDITIONS A. Construction and procedures shall be in conformance with the latest OSHA, NFPA, Interim Life Safety Measures (ILSM) and Infectious Control (IC) Standards of Hospital Environment. B. Any Utility support/outages required to accomplish the renovation of C4 (General Surgery), E2 (Eye Clinic) F2 (MSU) and J201 (ENT) Waiting Room Areas shall be scheduled and coordinated with the BOS Contractor and prior notification shall be made to Naval Hospital Facilities Management Department. Any Cost for Utility Support shall be the responsibility of the Contractor. IIICONTRACT AWARD A. Pre-construction: Contractor will meet with the Facilities Management Department (FMD) representatives on elaborative efforts and means of the work or job to be accomplished. B. ILSM: A meeting on Interim Life Safety Measures (ILSM) will be scheduled and conducted upon completion of Pre-Construction meeting. Representatives and officials from Naval Hospital Safety, Fire Safety, Infection Control, and Security will also be present at this meeting to ensure hospital requirements are adhered to and maintained at all times during construction. IVCONTRACT/WORK ACCEPTANCE A. The Facilities Project Manager will perform a PRE-FINAL/quality assurance inspection. All discrepancies identified during pre-final will be re-worked and performed accordingly. B. A FINAL INSPECTION will be performed, cleared, and approved by Facilities Management Department before contractor is relieved from work or contracted job. ________________________________________ Point of Contact: Albert C.
