Description
The Department of Veterans Affairs, Procurement and Business Division, Contracting Office, VA Medical Center, located at 619 South Marion Avenue, Lake City, Florida 32025, intends to enter into a contract on the basis of full and open competition for a special VA Sponsored Rural Health Initiative for primary care, basic mental health services, and preventive medical care services for eligible veterans enrolled with the Department of Veterans Affairs (VA) North Florida/South Georgia Veteran Health System. The VA intends to award a Firm Fixed Price contract for primary care services for eligible veterans living in and around Taylor County, Florida. The Contractor shall furnish health care providers, medical facilities, equipment and supplies, emergency-fill pharmaceutical prescriptions, and administrative functions to fulfill the level of care and range of services necessary to meet the primary medical needs of enrolled patients. The Contractor shall also be responsible for the administration and management of all aspects of this contract and the health plan covered there under, including responsibility for all employees, agents, subcontractors and any other entity acting for or on behalf of the Contractor. Based on the Independent Government Cost Estimate, the anticipated dollar value of the proposed firm fixed price acquisition is between $100,000 and $250,000 for a 12-month base period, with three (3) 12-month option periods. The estimated date of award for this action is October 01, 2009. The statutory authority permitting this action is 38 U.S.C. 8153 as implemented by the Federal Acquisition Regulation (FAR) Subpart 6.100 entitled "Full and Open Competition". The North American Industrial Classification (NAICS) for this effort is 621111. For successful performance of this contract, the Contractor shall continuously meet or exceed Joint Commission Standards, follow all applicable NF/SGVHS policies and procedures and comply with all applicable privacy and confidentiality statutes and regulations of the Health Insurance Portability and Accountability Act (HIPPA).
