- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR 12.6, as supplimented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is 1057867. The solicitation is issued as a Request for Quote (RFQ). Quatations are due 8/28/09, by 3:00 PM Central Daylight Time. The offers are due by email, mail or fax. The RFQ must reference solicitation number 1057867. The solicitation document and incorporated provisions and clauses are those in effect through Federal Circular (FAC) 2005-36. The associated NAICS code is 511210 Software Publishers. The Food and Drug is soliciting for the following item: Viocare lifestyle assessment database to establish a custom website and database for NCTR that incorporates tools for diatary and physical activity assessments using validates food frequency questionnaires based on University of Minnesota food database system and standardized and validated activity questionnaires. External food survey assessment tools should also be included in the customized database system. Included in the physical activity assessments would be the ability to upload OMRON pedometer data with step challenges. The customized system should have the ability to track changes in nutrient intakes and activity levels, and adherence to wellness plans. The data should be exportable in various formats for uploading to other databases. This customized system should be managed and maintained for a 12 month period with options for renewal each year. Responses to this soliciatation shall include both technical and cost information: Technical will receive paramount consideration over cost information. The Government reserves the right to award without discussion. The quote shall include all specifications, descriptive material, literature, brochures and other information; the government is not responsible for locating or securing any information not included with the offerors quote. Offerors must include a complete copy of the provisions at FAR 52.212-3. Offeror Representations and Certifications - Commercial Items with its offer.
