This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation W81K00-09-T-0374 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-26. This acquisition is unrestricted and all responsible sources may submit a quotation under NAICS code 325413 and the business size standard is 500 employees.The requirements in this solicitation are: Biological Products for Diagnostic testing; CLIN 0001 ADD Herpes Buffer Im in Normal Saline # 17-737E (16 ea); Clin 0002 ADD Dulbeccos Phosphate Buffer # 17-512F (70 ea); Clin 0003 ADD Guinea Pig Red Blood Cells # 30-957J (52 ea); Clin 0004 ADD Refrigeration Fee per shipment if required; Clin 0005 ADD DHEM w/Glucose & Herpes Buffer # 12-709F (130 ea); Clin 0006 ADD L-Flutamine 200mm in normal saline # 17-605E (30 ea); Clin 0007 ADD GENTAMYCIN SULFATE 10mg/mL # 17-519L (14 ea); Clin 0008 ADD PENICILLIN-STREPTOMYCIN FUNGIZONE MIX # 17-745H (80 ea); Clin 0009 ADD FETAL BOVINE SERUM 100mL # 14-501E (25 ea) that these reagents are used for diagnostic testing results. MFG LONZA WALKERSVILLE FY10, REQMT WILL BE CALLED IN AS NEEDED BY COR: ORDERING OFFICER. - ACCEPTANCE OF GOVT PURCHASE CARD AS Payment Tool will be required. PERIOD OF PERFORMANCE: 01 OCT 2009 THRU 30 SEP 2010. SITE: Ft Sam Houston TX 78234 FOB -DEST Vendors shall comply with FAR Clause (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/. Delivery, 7 -10 days after receipt of order, shipping shall be FOB - DEST FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Vendors shall include a completed copy of FAR provision 52.212-3 with their offer.
