Date: 8-26-2009Closing Date: 9-4-2009 From: Louis Stokes Cleveland, Department of Veterans AffairsVISN 10 Contracting Activity10000 Brecksville Road, Brecksville, OH 44141Davina Williams, Administrative Contract OfficerEmail: Davina.Williams@va.gov The Louis Stokes Cleveland VA Medical Center is seeking potential sources to provide a machine for performing Vitreoretinal surgery. The equipment must have the following features: a)High Speed Vitrectomy probe that functions at 5000 cuts per minute for 25, 23, and 20 gauge surgeries.b)The surgeon controls "duty cycle" and has 3 options: port biased open, port biased closed and 50/50. With this feature the surgeon has the ability to control flow of the vitreous without changing cut rate or vacuum.c)Integrated Pressurized Infusion System and Intraocular Pressure Control that constantly monitors and provides control of the infusion pressure resulting in a more stable IOP within +/- 2mm Hg.d)Provides venture based vacuum up to 650mm Hg.e)Auto Gas Fill - a single person sterile process to automatically purge and fill a syringe with undiluted C3F8 and SF6 gases.f)Programmable footswitch that allows the surgeon preferences for all modes and sub-modes independently.g)Simultaneous control of pneumatic scissors and forceps from the footswitch.h)17" articulating graphic user interface that is extendable and rotate-able through 180 degrees.i)Infusion Bottle Warning System that warns of low level in BSS bottle, as well as, bottle empty condition. j)Customized "end of case" reporting for consumables and metrics that can be wirelessly transmitted to a printer. This is a sources sought synopsis only. The purpose of this synopsis is to gain knowledge of potential qualified sources and their size classification (service disabled veteran owned small business, veteran owned small business, hubzone, 8(a), small disadvantage business, small business, or large business) relative to NACIS 221310 (size standard of 6.5M per annum). Responses to this synopsis will be used by the Government to make appropriate acquisition decisions. Responses to this notice should include company name, a service capability statement, three customer references to which you have provided similar services to, copies of applicable licensing, product catalog, address, point of contact, and the size of your business pursuant to the following questions: (1) Is your business large or small (2) If small, does your firm qualify as a small, emerging business, or small disadvantaged business? (3) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act? (4) Is your firm a certified hubzone firm? (5) Is your firm a woman-owned or operated business? (6) Is your firm a certified Service-Disabled Veteran Owned Small or Veteran Owned Small Business? Responses to this notice shall be e-mailed.
