- Equipment List & condition This is a combined Synopsis/Solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. THIS ACQUISITION IS SET-ASIDE FOR SMALL BUSINESS. The U.S Environmental Protection Agency (EPA) Region 1 Office intends on awarding a firm fixed price order which provides all service and maintenance on-site for the various existing Government Owned Equipment as described in attachment 1. Provide Service and Maintenance for the equipment listed in attachment 1 as follows: (a). Telephone support with 2 hour call back Monday - Friday 8:00AM - 5:30PM excluding Holidays. (b). Unlimited Emergency Service visits with two (2) working days on-site response time which includes all trouble shooting, repair, replacement parts, and travel expenses. (c). Preventive Maintenance three (3) visits per year. Potential vendors are required to provide proof in writting that they have performed service and maintenance on similar make(s) and model(s) of equipment as identified in attachment 1, within the past three (3) years. The EPA intends on awarding an order for the following NINE (9) Month Period of Performance, 10/01/2009-6/30/2010. The planned NAICS code is 811219 and the size standard is $7.0 million. Interested parties who can meet the above requirements may submit a written quotation by e-mail which includes information regarding experience on similar makes and models as stated above and the total price for all of the equipment in attachment 1, no later than September 11, 2009, 4:30pm EDT, in support of this Request for Quote (RFQ). Submit your response to the Point of Contact listed in this Synopsis/Solicitation. No telephonic quotes will be honored. Quotes submitted after the closing date of September 11, 2009, 4:30pm EDT, will not be considered. The following Federal Acquisition Regulation (FAR) Provisions and Clauses as applicable in FAC 2005-36 apply: The provision at 52.212-1, Instruction to Offerors - Commercial Items, applies to this acquisition. The provision 52.212-2 Evaluation -Commercial Items. The following factors shall be used to evaluate offers: For the initial blank section insert the following: First - Vendors experience on similar make(s) and model(s) of equipment as identified in attachment 1, within the past three (3) years. Offerors meeting the above experience requirement will then be evaluated on the lowest quoted price for all the equipment listed in attachment 1. For the final blank section insert the following: Not Applicable. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, with its offer. Please note: An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov. If an offeror has not completed the annual representations and certifications electronically at the ORCA website, the offeror shall complete only paragraphs (c) through (m) of this provision. The clause at 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition.
