- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; bids are being requested and a written solicitation will not be issued. The solicitation number is VA-528-09-RQ-0228 and is issued as an invitation for bids (IFB), unless otherwise indicated herein. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-34. The associated North American Industrial Classification System (NAICS) code for this procurement is 423450 with a small business size standard of 100.00 employees. This requirement is unrestricted and only qualified offerors may submit bids. The solicitation pricing on www.FedBid.com will start on the date this solicitation is posted and will end on 2009-09-08 12:00:00.0 Eastern Time or as otherwise displayed at www.FedBid.com. FOB Destination shall be Bath, NY 00000 The VHA Buffalo Medical Center requires the following items, Exact Match Only, to the following: LI 001, Fixed 5 week rental of mobile CT scanner (09/20/09 - 11/01/09). 1. GENERAL REQUIEMENTS:a. The Contractor shall provide the delivery/set-up of a fully functional mobile unit before the removal of the existing CT and the installation of the new 64 slice CT with no interruption of current CT in-house services being provided 2. STATEMENT OF WORK: a.Contractor will provide a mobile Philips Brilliance 16 slice scanner. (Background Justification: VAMC Bath currently owns a Philips Brilliance 16 slice scanner. To rent any other similar CT Scanner would require a 40-hour training session for six staff members. Such action would be costly and time consuming and is considered unreasonable for a short-term rental agreement). b. Contractor shall deliver and set up scanner on site at the Bath VAMC on 09/20/09 c.The contractor shall provide preventative maintenance and emergency repairs. d.This mobile unit shall be available seven days (24/7) per week. It shall be on site to cover any scheduled, emergency, inpatient or outpatient exam e. This mobile unit shall hook up to the existing electrical outlet that is provided on property. This mobile unit shall be on site ready to use when de-installation of the existing CT machine begins. f. Scanner will need to be on site for approximately 5 weeks until 11/01/09. g.Contractor shall provide 24 hour access to service call center. h. Contractor shall provide an up time of 95 percent. i. Mobile unit shall have a power injector that will accommodate Mallinckrodt 125ml syringes. j. Mobile unit shall have ability to send images to DICOM (Philips PACS) k. Mobile unit shall also have: Dual monitor technologist workstation, Modality worklist, Phone connection, Scanner table with a minimum 450 lbs weight capacity, and Lift on trailer able to accommodate patient stretcher and a lift capacity of 700lbs minimum., 1, EA; LI 002, Option for Additional 5 weeks - quote on a weekly basis for a total of 5 additional weeks, 5, weeks; For this solicitation, VHA Buffalo Medical Center intends to conduct an online competitive reverse auction to be facilitated by the third-party reverse auction provider, FedBid, Inc. FedBid has developed an online, anonymous, browser based application to conduct the reverse auction. An Offeror may submit a series of pricing bids, which descend in price during the specified period of time for the aforementioned reverse auction. VHA Buffalo Medical Center is taking this action in an effort to improve both vendor access and awareness of requests and the agency's ability to gather multiple, competed, real-time bids. All responsible Offerors that respond to this solicitation MUST submit the pricing portion of their bid using the online exchange located at www.FedBid.com. There is no cost to register, review procurement data or make a bid on www.FedBid.com. Offerors that are not currently registered to use www.FedBid.com should proceed to www.FedBid.com to complete their free registration. The selected Offeror must comply with the following commercial item terms and conditions. FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. The selected Offeror must submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items. FAR 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The following FAR clauses in paragraph (b) of FAR clause 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items, will apply: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34. CCR Requirement - Company must be registered on Central Contractor Registration (CCR) before an award could be made to them. If company is not registered in CCR, they may do so by going to CCR web site at http://www.ccr.gov. 52.211-11 Liquidated DamagesSupplies, Services, or Research and Development. As prescribed in 11.503(a), insert the following clause in solicitations and contracts: Liquidated DamagesSupplies, Services, or Research and Development (Sept 2000) (a) If the Contractor fails to deliver the supplies or perform the services within the time specified in this contract, the Contractor shall, in place of actual damages, pay to the Government liquidated damages of $10,000 per calendar day of delay [maximum penalty: 5 days and $50,000]. (b) If the Government terminates this contract in whole or in part under the DefaultFixed-Price Supply and Service clause, the Contractor is liable for liquidated damages accruing until the Government reasonably obtains delivery or performance of similar supplies or services. These liquidated damages are in addition to excess costs of repurchase under the Termination clause. (c) The Contractor will not be charged with liquidated damages when the delay in delivery or performance is beyond the control and without the fault or negligence of the Contractor as defined in the DefaultFixed-Price Supply and Service clause in this contract.
