- Description
- The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for Optometry care and consulting at the IHS, PHS Hospital in Fort Yates, North Dakota. The period of performance will be October 12, 2009 through September 30, 2009. The Contractor shall perform according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-10-022, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-10-022 and provide a postal mailing address, email address, and/or fax number and indicate how the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be September 11,2009. The closing date of the RFQ package is September 28,2009.
