Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. M6700109Q0158 is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-32, effective 14 May 2009 and DFARS Change Notice (DCN) 20090901. The proposed acquisition is a 100% small business set-aside. The associated North American Industrial Classification System (NAICS) code for this requirement is 611710. To meet the small business size standard a concern's annual receipts must not average more than $6.0 million for their latest three completed fiscal years. A firm-fixed price contract will be awarded for the requirement that is described as follows: The purpose of this request for quote is to procure the following services: contractor shall provide 12 months of non-personal youth ministry services as a part of the Command Religious Program (CRP) and adequate oversight to ensure compliance with contract requirements. This is an ecumenical youth ministry program and the primary goal of the program is to foster and strengthen faith commitments through fellowship, activities, youth meetings, events, retreats, camps and mentoring programs involving dependent military youth living aboard Marine Corps Base (MCB) Camp Lejeune NC. A detailed description of the Government's requirement is located at Attachment #2. The provision at 52.212-1, Instructions to Quoters - Commercial Items applies to this acquisition. 52.212-2, Evaluation - Commercial Items. Evaluation: Responses to this RFQ will be evaluated and awarded based on best value to the Government. Evaluation criteria are: technical capability, past performance, and price. Technical Capability: Resumes are required and will be evaluated based on the areas listed in the Statement of Work (SOW), at IV Technical Requirements - 1.1 through 1.10, and 2.1 Minimum Education, Qualifications, and Experience Requirements. (Attachment #2 SOW). Pricing: Prices shall be firm fixed priced monthly prices.Period of Performance: The period of performance shall be for a twelve-month base period, 01 September 2009 or date of award (whichever is later) through 31 August 2010, with two 12-month option periods to be exercised at the Government's discretion.Past Performance: Quoters shall provide at least three (3) relevant past performance references with their quote. Reference information shall include agency name, contract number, dollar amount, point of contact (include phone number), for which your firm has provided the same or similar items. Quoters are to include a completed copy of the provisions at FAR 52.212-3 Offeror Representations and Certifications -- Commercial Items. If the offeror has completed the annual representations and certifications electronically at http://orca.bpn.gov only paragraph (k) of this provision shall be completed. As a condition of receipt of this contract, the Contractor agrees to implement the Department of Defense web-based software application, called Wide Are Workflow Receipt and Acceptance (WAWF-RA). This application allows DoD vendors to submit and track invoices and Receipt/Acceptance documents electronically. Quotes shall include (1) pricing information; (2) technical information - resumes addressing the tasks and requirements in the SOW; and (3) relevant and acceptable past performance information, providing references that demonstrate the contractor's experience in performing work of a similar type, size, volume and complexity as stated in this requirement.
