- SF 1449 (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. 1. (ii) Solicitation #RFQP04170900005 (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 36. (iv) This procurement is set-aside for Small Business. (v) Subsistence items: Dairy (vi)Provide a fixed price for the items required.. (vii) Delivery to begin October 01, 2009 until September 30, 2009 to FCI, Milan, Michigan, 48160 (Destination) and will be accepted from 7:30 am to 1:30 pm Monday - Thursdays. All drivers need to complete a security background check prior to entry, to include a valid driver's license. Delays for delivery due to security concerns of facility and inherent weather will be incurred and are not reimbursable. All deliveries must be coordinated prior to shipment. No passengers, pets, weapons, or tobacco products will be allowed within the secure perimeter. No sideways pallets. No forklift is available for unloading of pallets. Vendor shall pass all requirements to transportation company. Failure to adhere to regulations will be cause for rejection of shipment. (viii) The provisions at 52.212-1, Instructions to Offers-Commercial, applies to this acquisition. (ix) 52.212-2, Evaluation - Commercial Items, - Price (x) Offerors are to provide a copy of 52.212-3, Offeror Representations and Certifications-Commercial Items, with offer. (xi) Clause 52.212-4 Contract Terms and Conditions-Commercial Items, applies to this acquisition. 52.212-5, Contract Terms and Conditions required to Implement Statutes or Executive Orders-Commercial Items applies to this acquisition.
