- This requirment has a short suspense due date for quotes to be received of 5 September 2009. This requirement is procured on an unrestricted basis with preference given to HUBZONE, Service Disabled Veteran Owned Small Business, 8(A), Small Business. All business type are encouraged to participate. All offerors interested in receiving a copy of the solicitation must request it by e-mailing Timothy Pighee at tim.pighee@ar.ngb.army.mil or be phone at 501-212-4401. Following are the requirements needed for approximately 318 Soldiers to be conducted 12-13 September at the National Guard Armory located at 305 South Main Street, Searcy Arkansas, 72143-9109: Dental Exams will include the following: Bitewings Panoramic Radiography, if required. All x-rays will be digital and loaded into DENCLASS within 5 days after the event. All dental exams will be loaded into DENCLASS and the results from the exam will be annotated in the dental record. Dental exams will be conducted with dental explorer and mouth mirrors. The Periodic Screening Assessment (PSA) will be annotated as a part of the dental exam also. Dental exams classification will be annotated by the following: DENTAL CLASS 1- Patient has good health and is not expected to require dental treatment of re-evaluation for 12 months. DENTAL CLASS 2- Patient has some oral conditions, but you do not expect these conditions to result in dental emergencies within 12 months if not treated, (i.e., requires prophylaxis, asymptonatic caries with minimal extension into dentin, edentulous areas not requiring immediate prosthetic treatment). DENTAL CLASS 3- Patient has oral conditions that you do expect to result in dental emergencies within 12 months if not treated. DENTAL CLASS 4 - Patient requires a current (within 365 days) dental examination in or to determine if they are Class 1, 2 or 3. A missing panoramic radiograph is also a dental class 4 condition. PERIODIC HEALTH ASSESSMENTS (PHS) Contractor will conduct basic Periodic Health Assessments to include the following: Visual screening Immunizations as identified in the Medical Protection System (MEDPROS): Hepatitis A or B series; Tetanus; Diphtheria; Influenza, and other as identified by the Contracting Officers Representative (COR). NOTE: GOVERNMENT WILL PROVIDE ALL THE ABOVE SERIUMS: Conduct Medical Screenings on all Soldiers to include height, weight, blood pressure, ECG (40 and up). Hearing screening utilizing the required equipment with Defense Occupational and Environmental Health readiness System (DOEHRS). Lab, only if needed, and as identified in the Medical Protection System (MEDPROS), HIV, DNA, Blood Type, CVSP (over 40 blood draw), G6PD, PAP smear screening (female only) and rectal and stool guiac for females 38 and over. NOTE: THERE WILL BE NO NEED FOR A RECTAL EXAM FOR MALES.
