Description
This is not a solicitation announcement, this is a sources sought notice only. The following describes an overview regarding the requirements for veteran patient focused advisory and assistance services. This sources sought neither constitutes a request for quote or proposal, nor does it restrict the Government to an ultimate acquisition approach. This sources sought should not be construed as a commitment by the Government for any purpose. It is a request for information from sources that may be able to provide the services described herein. The Veterans Integrated Service Network 11 (VISN 11), a network of eight (8) Veterans Affairs Medical Centers located in Illinois, Indiana, and Michigan is seeking potential sources to provide advisory and assistance services to improve veteran patient centered care, patient education and satisfaction with the Medical Centers. The contract requirements will include providing training manuals, sample policies and procedures, introductory information sessions on veteran centered approaches to care, conference strategy calls, leadership retreat sessions, organizational progress assessments, assessment recommendations to each facility, and detailed project management plans for each facility to implement and maintain a veteran patient focused care model. Coordinator orientation sessions shall also be provided for training and mentoring of the designated coordinators at each VISN 11 facility. The contractor shall provide VISN Leadership and staff retreats and training in implementation and practices of the veteran centered care model. Leadership retreats will include members of VISN 11 management and administration. Staff retreats shall also include facilitator training, coaching, and provide for feedback and fine-tuning of new policies and procedures required for the Contractor's the veteran centered care model. The contractor shall also provide advisory and assistance telephone sessions with VISN staff to assist in fully implementing and maintaining the veteran centered care model. The Contractor shall provide follow-up progress assessments to determine the effectiveness of the veteran centered care model and its impact on patient care. The progress assessments shall include a facility review, focus groups, and meetings with the veteran-centered care coordinators. Contractor shall then provide initial feedback and recommendations for implementation to leadership and to Executive Leadership Committee, and final report documentation. The assessment recommendation report shall be presented to staff and executive leadership at each facility and shall provide an analysis of collected data with recommendations. The contractor provided Project Management Plan shall detail the project team, schedule of activities, and identify the contractor staff to provide further assessment and advisory and assistance services. The proven ability to collect and analyze progress assessment information and tailor facility specific plans for improvement will be critical to meeting these requirements. This must also include experience in centering care around the patient and increasing their involvement and education about the care provided and the environment it is provided in. Submitted responses to this notice must provide information including a list of similar contracts that have been performed within the past five years including name and phone number of contact person for each contract. Please provide any information that will demonstrate your company's ability and expertise to perform the services described herein.
