Description
GENERAL INFORMATION The 42nd Contracting Squadron, 50 LeMay Plaza, Bldg 804, Maxwell AFB, AL has a requirement to purchase items as described in the Schedule of Supplies/Services. This is a streamlined combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only notice. Quotes are being requested and awritten solicitation will not be issued. This requirement is issued as a request for quote (Reference-Number FA3300-09-Q-STIMU09). The solicitation document and incorporated provisions and clauses are those in effect the Federal Acquisition Circular 2005-36 (11 Aug 2009) and the Defense Federal Acquisition Regulation Supplement (DFARS) change notice 20090825. It is the contractor's responsibility to become familiar with applicable clauses and provisions. This purchase will be 100% Small Business Set-Aside. A Firm Fixed Priced Award will be made in accordance with the following the Schedule of Supplies: NAICS Code: 334510; size standard: 500 employees The Defense Priorities and Allocations System rating is C9E. IMPORTANT NOTE: THE GOVERNMENT IS SOLICITING FOR "OPEN MARKET" QUOTES ONLY. GENERAL SERVICES ADMINISTRATION (GSA) QUOTES WILL NOT BE CONSIDERED. NATURE OF ACQUISITON: SROTC/JROTC has a requirement for Biphasic Defibrillators and batteries. The Schedule of Supplies/Services provides all items and quantities required. This procurement will be awarded under Simplified Acquisition Procedures. SCHEDULE OF SUPPLIES/SERVICES: This acquisition is for the purchase of the following items: CLIN 0001AA: Biphasic Defibrillator for EMS Activities; Features must include: a) ECG Monitoring with patient connection of 5-lead and 12-lead ECG cable, Paddles and MFE pads, Input Protection, Implanted Pacemaker Spike Display, Bandwidth, Lead Selection, ECG Size, Heart Rate, Heart Rate Alarm, 1 Volt ECG Out, Display Format, Smart Alarms. b) Defibrillation; Charge Time, Energy Display, Multi-Function Electrode, Built-In Defibrillator Tester, Multifunction Electode c) Manual (Adult Mode); Energy Selection, Synchronized Mode, Paddles, Charge Controls d) Advisory (AED Function); Advisoty Function, Shockable Rhythms, Charge Controls, Energy Selection e) Pacemaker; Type, Pulse, Output Protection f) Accessory; Non-Invasive Blood Pressure, Pulse Oximeter function g) Voice Prompts h) Visual Prompts; Quantity: 3 each CLIN 0002AA: Battery; compatible with defibrillator quoted, Quantity: 3 each CLIN 0003AA: Battery Charger; Multicharger, compatible with battery and defibrillator quoted; Quantity: 1 (one) each CLIN 0004AA: Defibrillator Protective Case; compatible with defibrillator quoted; Quantity: 3 each ALL CLINs ARE FOB DESTINATION. Delivery is 30 days after receipt of order. QUOTATION PREPARATION INSTRUCTIONS To ensure timely and equitable evaluation of quotes, offerors must comply with instructions contained herein. Quotes must be complete, self-sufficient, and respond directly to the requirements of this RFQ. a. The offeror must the accede to all solicitation requirements and the terms and conditions by submitting the following statement with your response to this solicitation: "I accede to all of the requirements of this synopsis/solicitation, including specifications (CLINs 0001AA - 004AA), and terms and conditions." b. Provision FAR 52.212-1, Instructions to Offerors: Commercial, applies to this acquisition c. Offerors must include a completed copy of the provision at FAR 52.212-3, Alternate I, Offeror Representations and Certifications -Commercial Items DFARS 252.212-7000, Representations and Certifications-Commercial Items and DFARS 252.225-7000, Buy American Act Certificate with quotation. d. Offerors must provide a unit price and extended price for all items referenced in the "SCHEDULE OF SUPPLIES/SERVICES". e. Interested parties capable of providing the requirement must submit a written quote to include discount terms, tax identification number, cage code, and DUNS. YOU MUST INCLUDE THE BRAND, MAKE AND MODEL OF THE ITEMS THAT YOU QUOTE. To be eligible to receive an award resulting from this RFQ, contractors must be registered in the DoD Central Contractor Registration (CCR) database, no exceptions. A single award will be made in the resultant contract. BASIS FOR AWARD This is a competititive best value acquisition utilizing simplified acquisition procedures authorized by FAR 13.5, "Test Program for Certain Commercial Items." The offeror by acceding (see quotation preparation and instructions) to all of the requirements of this synopsis/solicitation, including specifications (CLINs 0001AA - 0004AA for the base period and all option years), terms and conditions will be determined technically acceptable. Award will then be based on price.
