- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0487 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. This acquisition is set-aside 100% for small business under NAICS code 339112 and the small business size standard is 500 employees. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: 1ea ENT Fusion Navigation Instrument Set, PN: 999062526; 1ea Microdebrider, PN: 999035941; 1ea Wireless Mouse, PN: 999060303. Mfr: Medtronic Vendors offering other than the brand name product shall meet or exceed the following Salient Characteristics: 1.Must be able to save and load user specific settings 2.Must be footpedal free 3.Must have video in/out 4.Must have reuseable instruments with multi-instrument tracking and surgeon specific settings 5.Must be new, not refurbished Delivery, within 20 days after receipt of order, shipping shall be FOB destination to General Leonard Wood Army Community Hospital, 126 Missouri Ave, Ft Leonard Wood, MO 65473. FAR provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Vendors shall comply with FAR 52.211-6, Brand Name or Equal, if submitting other than the Brand Name product. Offers will be evaluated according to 52.212-2. Evaluation factors at 52.212-2 (a) are price and (b) conformance to salient characteristics outlined in this solicitation. (Lowest Price Technically Acceptable) Vendors shall include a completed copy of FAR provision 52.212-3 as outlined therein.
