- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. W81K00-09-T-0482 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. This acquisition is unrestricted under NAICS code 334510, Size STD 500. FAR provisions and clauses in this solicitation may be viewed at http://www.arnet.gov/far/. DFARS clauses may be viewed at http://www.acq.osd.mil/dpap/dars/dfarspgi/current/index.html. The requirements in this solicitation: 1.1ea iLab Ultrasound Imaging System to include Installation. Part#H749iLab120INS0. Mfr: Boston Scientific Vendors offering other than the brand name product shall meet or exceed the following Salient Characteristics: 1.Must have DICOM compatibility with CV-PAC archive/storage (Vericis). 2.Must have dynamic review capability (cross-sectional and long axis views). 3.Must permit rapid measurements of diameter and length real-time; adjust sizing within each image. Post case: area, length. 4.Must have pseudo colorization to show blood flow, plaque and lumen. 5.Must have magnification/zoom feature. 6.Must have patient anonymization. 7.The monitor in lab must be hung on an angiography boom. 8.Must be simple operation. Delivery date is within 2 weeks after receipt of order. Place of delivery and acceptance is Brooke Army Medical Center, 3851 Roger Brooke Dr, Ft Sam Houston, TX 78234-6200. FOB destination is preferred. FAR provision 52.212-1, Instructions to Offerors Commercial, applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/. Confirmation of CCR registration will be validated prior to award. Vendors shall comply with FAR 52.211-6, Brand Name or Equal, if submitting other than the Brand Name product. Offers will be evaluated according to 52.212-2. Evaluation factors at 52.212-2 (a) are price and (b) conformance to salient characteristics outlined in this solicitation. (Lowest Price Technically Acceptable) Vendors shall include a completed copy of FAR provision 52.212-3 ALT I as outlined therein.
