Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Prospective offerors are encouraged to view this site frequently for any amendment to the solicitations. This solicitation is issued as a Request for Proposal (RFP), solicitation number VA-259-09-RP-0432. The George E. Wahlen, VA Medical Center, Salt Lake City, Utah, has a requirement for a S-23 Chlorine Dioxide Generator Pumping System. Statement of Work: 1.Provide and install a 30 gallon double containment and water containment and water solutions S-23 chlorine dioxide generator pumping system into the main domestic water line in Building 43. Work to include a check valve. System to contain batch tank, pump with VFD (vacuum fluorescent display) and inverter duty motor, Signet paddle wheel or magmeter sensor with flow transmitter. Start up and training to be included in price. 1 Job $_____________________ 2.Provide and install equipment, monitoring of operations and solutions for S-23 site-generated CIO2 System 3 precursor chemical (hydrochloric acid, sodium chlorite and sodium hypochlorite) 1 Job $_____________________ This solicitation document and incorporated clauses and provisions are those in effect through Federal Acquisitions Circular 2005-36. Prospective Offerors must also respond to 52.212-2, Evaluation of Offers. 1. Technical Capability. Contractor must be regularly established in the business called for in this solicitation. 2. Past Performance. Offeror shall provide a minimum of 3 references, including name, address, telephone number and point of contract for whom the same or similar type work has been performed. 3.Price. Award Statement. Technical Capability and Past Performance, when combined are of greater importance when compared to price. All prospective offerors desiring to submit proposals in response to any Federal Agency must obtain a Dun & Bradstreet number (DUNS) at www.dnb.com. There is not cost to obtain this number. Offerors must also register with the Central Contractor Registration Site at www.ccr.gov. The North American Industry Classification System code for this procurement is 423720; Small business size is 100 employees. This acquisition is 100% set-aside for small business.
