Description
DescriptionThis is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. It is the intent of the Syracuse VA Medical Center located in Syracuse, NY, to negotiate a sole source procurement with Formulary Productions, LLC, 6263 Poplar Avenue, Suite 1133, Memphis, Tennessee 38119 to provide a customized online formulary, pharmacy communications tool and medication safety portal (description and capability requirements listed below.) The purchase will be negotiated sole source based on most efficient organization criteria and as there is only one responsible source and no other supplies or services will satisfy agency requirements; therefore, full and open competition is determined inappropriate for this acquisition. This requirement is to provide for Base Year and 2 Option Years: 1 Base Year (Oct 1, 2009 - Sept 30, 2010). Site Build Fee, including Custom Website Development and Training, Annual Site License including ongoing Customer Support and Product Enhancements for VISN2 Medical Centers. Total Annual Cost ____________ 2 Option Year 1 (Oct 1, 2010 - Sept 30, 2011). Annual Site License including ongoing Customer Support and Product Enhancements for VISN2 Medical Centers. Total Annual Cost ____________ Option Year 2 (Oct 1, 2011 - Sept 30, 2012). Annual Site License including ongoing Customer Support and Product Enhancements for VISN2 Medical Centers. Total Annual Cost ____________ Description and capability requirements: Provide a customized formulary website, hosting VISN2'S approved formulary and unique medication safety information; extensive training and customer support, including homepage development; password-protected Editor tools to update and website maintenance; services to host site in a secure environment. Must comply with and exceed JCAHO requirements for formulary management; publish, manage and update approved formulary and medication information simply and efficiently, making it easily accessible to approved medical staff and other users through the Internet, intranet, PDA downloads, or print; search and view medications in the formulary by generic or brand names, by whole or partial word, by alphabetical or AHFS therapeutic category or by special alert listings; publish pharmacy Adverse Drug Reaction forms, guidelines and policies; pharmacy services and phone numbers; unlimited news stories, critical medication safety information, backorders, shortages, discontinuations, recent FDA approvals, etc.; improve medication safety by providing timely information such as recalls, look-alike/sound-alike drugs, High Alerts, FDA safety alerts, and Black Box warnings; access the a library of best practices and information; link to subscription and free drug information providers on the Internet, drug order forms, clinical calculators, customized discussion boards, "mail to" sources, ISMP and other important medication safety information. No other sources can provide the customized online formulary, pharmacy communications tool and medication safety portal. The Contracting Activity will consider offerors' quotations submitted by Sept 17, 2009. This notice is for information only and may represent the only official notice of such solicitation/synopsis, however, the Government will consider all proposals received on or before due date identified below. A determination by the Government not to compete this proposed procurement based on the responses to this notice is solely within the discretion of the Government. The provision at 52.212-1, Instructions to Offerors - Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification - Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its proposal. A copy of the provision may be attained from http://www.arnet.gov/far/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions - Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items, applies to this acquisition. The offeror shall also provide its Commercial and Government entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of the information added to CCR. To register, go to http://www.ccr.gov/.
