- Justification and Approval for Specific Make and Model Basic Equipment List The Defense Information Systems Agency (DISA) has a requirement for Black Box and Avocent hardware to support mission requirements associated with CENTRIXS. The products and quantities are attached. Offerors responding to this requirement should propose on an "all or none" basis. Partial quotes are unacceptable. Quotes will be accepted in the open market contract only. The Government will award a single purchase order to the responsible offeror whose offer conforms to the requirements as stated in this request for quotation (RFQ) and is most advantageous to the Government based on the lowest price. This acquisition is firm-fixed-price (FFP) and solicited on a brand name basis in accordance with FAR 6.302-1. The Justification and Approval form is attached. Configuration and compatibility issues justify the use of brand names. Only qualified offerors may submit a quote. New equipment ONLY, NO remanufactured products, and NO "gray market". Quote must be good for 30 days after due date. Quote per specifications requested. If an item has been discontinued, or is end of life, annotate quote, referencing as such, then quote an equal or better replacement for the item. Quote is to include all fees, including shipping or freight costs. Items will be shipped to the following address: DECC Pacific Attn: Mark O'Lague 1942 Gaffney St. Suite 200 Bldg 475, 3rd Floor Pearl Harbor, HI 96860-4582 Upon acceptance of this award your company certifies it is a Black Box and Avocent Manufacturer Authorized Reseller that has the certification/specialization level required by Black Box and Avocent to support both the product sale and product pricing, in accordance with the applicable Black Box and Avocent certification/specialization requirements. Your company shall warrant that the products are new, in their original box. Your company confirms to have sourced all Black Box and Avocent products submitted in this offer from Black Box and Avocent or through Black Box and Avocent Authorized Channels only, in accordance with all applicable laws and policies at the time of purchase. The following provisions/clauses in their latest editions apply to the award: FAR 52.203-13, Contractor Code of Business Ethics and Conduct; 52.204-9000, Points of Contact; 52.204-9001, Contract/Order Closeout - Fixed Price, Time & Materials, or Labor Hours; 52.211-6, Brand Name or Equal; 52.212-3, Offeror Representations and Certifications - Commercial Items; 52.212-4, Contract Terms and Conditions - Commercial Items; 52.212-5 (Deviation), Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items; 52.232-9000, Wide Area Workflow-Receipt and Acceptance (WAWF-RA), and 52.252-2 - Clauses Incorporated by Reference; DFARS 252.232-7003, Electronic Submission of Payment Requests; 252.232-7010, Levies on Contract Payments; 252-204-7004, Alternate A, Central Contractor Registration; Section 508 Compliance. The FAR clauses may be viewed at http://www.acqnet.gov/far. Contractors submitting quotes must be registered and active in the Central Contractor Registration (CCR) program (http://www.ccr.gov/). Contractors must also submit a completed copy of the provision 52.212-3, Offeror Representations and Certifications - Commercial Items. This can be obtained via the ORCA website (http://orca.bpn.gov). The North American Industrial Classification Systems (NAICS) code is 423430, with an associated business size of 100. Interested parties need to supply their DUNS number, Cage Code, Tax Identification number, business size, and delivery Time. All quotes must be submitted electronically via email no later than 2:00PM EST, 22 September 2009. Any amendment(s) issued to this combined synopsis/solicitation will be published on this website.
