Description
This a Combined Synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, Streamlined Procedures for Evaluation and Solicitation for Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only synopsis/solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation W91YU0-09-T-0081 is issued as a request for quotation (RFQ). The combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. The associated North American Industrial Classification System (NAICS) code for this procurement is 339115. The Size Standard for small business is 500 employees. This RFQ is being issued unrestricted. These items are for use at Madigan Army Medical Center, Tacoma, WA 98431. All interested and responsible Contractors shall provide an offer for the following Brand Name or Equal Products: Line Item 0001: 1 Each MP680300 Advanced Medical Optics, AMO Phacoemulsifier Sovereign Compact, includes the following in the quantities specified: (1) Each MP680135 - Remote; (1) Each MP680720 Foot Pedal Open Toe; (1) Each MP680P2 - I/V Pole; (1) Each MP680002 Cart; (1) Each OM0201011D Electric Oscillating Vitrectomy Cutter; (1) Each OM0504011 Limbal Infusion Needle; (1) Each OM0527011 Coaxial Irrigation Sleeve; (1) Each OM02908012 Sterilization Medium for EOVC; (2) Each OM2908011- Sterilization Tray 1-1/4 Medium; (2) Each 690697 Whitestar Phaco Hand piece; (1) Each CMPZ370049 Operator Manual English; (2) Each K106075 Bipolar Cord; (2) Each K121085 Forceps; (6) Each OPOIA2045D Solo I/A Hand piece. 0002: Shipping Charges (if not FOB Destination) FOB Destination is preferred to Madigan Army Medical Center, Tacoma, WA 98431. Pricing quoted shall include all applicable fees. Salient Characteristics for the AMO Phacoemulsifier Sovereign Compact are as follows: A device capable of phacoemulsification; wet-field bi-polar diathermy; automated irrigation and aspiration; reusable oscillating vitrectomy; state-of-the-art high vacuum technology in phaco mode up to 500mmHg; deliver microbursts of ultrasound in 4, 6 and 8 millisecond increments with varying mircrorest periods; allows for cold phacoemulsification of nuclear fragments; enables sleeveless bimanual microphacoemulsification; multitasking 32 bit microprocessor; microprocessor monitoring of intraocular-intra-operative fluidic control; true fluid venting system controlled by onboard computer combined with fully closed system; all parameters are fully programmable in all modes including: pumping speed, vacuum levels, occlusion mode and IV pole height as well as phaco powers in phaco mode; two diathermy sub-modes, 4 phaco sub-modes, 3 I/A sub-modes, 1 vitrectomy sub-mode; occlusion mode with independent aspiration flow rate and adjustable vacuum rise time, independent pre and post occlusion phaco attributes, independent pre and post occlusion phaco powers; device characterizes and auto-tunes each hand piece during set-up; offers choice of either 19 or 20 gauge, standard threaded phaco needles capable of and FDA labeled for 20 reuses; passive reflux; displays both equivalent phaco times and foot pedal times; offers disposable and reusable tubing options.
