- sole source doc This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is F3Z4539238A001; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-34 and Defense DCN 20090115. The North American Industry Classification System (NAICS) code is 621511. The business size standard is $13.5 million. The Federal Supply Class (FSC) is Q508. The Standard Industrial Classification (SIC) is 8071. Travis Air Force Base intends to award a sole source requirement for cell salvage system and autotransfusion on call 24/7 services. Performance period 1 Oct 09- 30 Sep 10. Justification for a sole source includes the following: Pacific Biomedical is the only company within the immediate area able to provide the 24/7 on call services. All responsible sources may submit a quotation, which shall be considered by the agency. The following provisions and/or clauses apply to this acquisition. Offerors must comply with all instructions contained in FAR 52.212-1 Instructions to Offerors -- Commercial Items (June 2008). FAR 52.212-2 Evaluation -- Commercial Items (Jan 1999): (a) The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered.
