Description

The Aberdeen Area Indian Health Service is currently seeking potential quoters for a Firm Fixed-Price, Non-Personal Healthcare Service Purchase Order for an Orthopedic Surgeon at the IHS, PHS Hospital in Fort Yates, North Dakota. Duty hours will be from 8:00am-4:30pm, 1-2 days per month. The period of performance will be from the date of the award through September 30, 2010. Services will be provided according to the Statement of Work. The unit price is an all inclusive cost which is to include travel, lodging, per diem, fringe benefits, federal, state and local taxes, plus all other costs pertinent to the performance of the purchase order. Requests for the Request for Quotations (RFQ) package, RFQ-10-031, shall be in writing either by email or fax. In the written request please reference RFQ number RFQ-10-031 and provide a postal mailing address, email address, and/or fax number and indicate how the RFQ package can be sent. The RFQ package will be released approximately fifteen (15) days after the date of this publication which will be October 1, 2009. The closing date of the RFQ package is October 15, 2009.
