- Attachment 3 - Past Performance Questionnaire Attachment 2 - Performance Plan Attachment 1 - Performance Work Statement Solicitation Travis AFB is seeking to lease a fully automated FDA approved blood bank analyzer utilizing ID-MTS Gel technology as well as a full line of traditional tube testing reagents that are used in conjunction with and are used as a secondary back up testing method for David Grant Medical Center, Travis AFB, CA. Performance will be 1 October 2009 through 30 September 2010 with four one-year Option periods. This will be Full and Open solicitation under the NAICS code 334516. Vendors must be actively registered with the Central Contractor Registration (CCR) at http://www.ccr.gov, Wide Area Work Flow (WAWF) at http://wawf.eb.mil, and at Online Representations and Certifications Application (ORCA) at http://orca.bpn.gov. The solicitation package will be posted on the Electronic Posting System (EPS), on 18 Sep 09. The closing date and time for submission of offerors will be on 25 Sep 09, 2:00PM (PST). The Government will not provide paper copies of the solicitation. Telephone, written, facsimile, or e-mail requests for the solicitation package will NOT be honored. Potential offerors are responsible for monitoring the EPS site for the release of the solicitation package, and any other pertinent information and for downloading their own copy of the solicitation package.
