Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice for the South Texas Veterans Health Care System (STVHCS) San Antonio and Kerrville, TX divisions. This announcement constitutes the only solicitation. A written solicitation will not be issued. Solicitation documents and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-36. NAICS 424210 with business size standard 100 employees. STVHCS has a requirement for a variety of radiopharmaceutical products for the period of October 1, 2009 thru December 31, 2009. This solicitation is being conducted in accordance with Federal Acquisition Regulation Part 13.5 Simplified Acquisition Procedures-Test Program. Prospective vendors are required to submit written proposal for the following radiopharmaceuticals based on a per unit basis cost. Pricing should be submitted on letterhead paper and address the following items: (1) Technetium-99m Sulfur Colloid,(2) Filtered Technetium-99m Sulfur Colloid,(3) Technetium-99m Pyrophosphate,(4) Technetium-99m MAA,(5) Technetium-99m DTPA/renal perfusion,(6) Technetium-99m DTPA/GFR determination,(7) Gallium-67 (8) Chromium-51,(9) Technetium-99m Glucoheptonate,(10) Xenon-133 gas/20 mCi,(11) Technetium-99m MDP,(12) Technetium-99m Pertechnetate unit dose,(13) Technetium-99m Choletec,(14) In-111/Tc-99m Ceretec WBC labeling,(15) Technetium-99m Cardiolite,(16) Technetium-99m,(17) Thallium-201,(18) I.V. Dipyridamole (up to 60mg),(19) I-123 Capsules / 100uCi,(20) I-123 Capsules / 200uCi,(21) Radiation Safety Services (full service),(22) I-131 Therapy Capsules,(23) In-111 Capromab Pendetide,(24) Technetium-99m Ceretec for brain scan,(25) Pyrophosphate vial,(26) Sincalide Vial,(27) Technetium-99m Mag-3,(28) Technetium-99m DMSA,(29) Sr-89 Chloride,(30) In-111 Pentetreotide (31) In-111 DTPA,(32) I-131 MIBG,(33) Delivery charges included at no charge,(34) Each additional delivery (35) Delivery Charges during non-routine hours,(36) Emergency RPh Call Out Fee Additional requirements: Contractor and all sub-contractors must be licensed by the Nuclear Regulatory Commission and be regularly established in the business of providing radiopharmaceuticals. Offerors must provide copies of licenses and certifications along with the offer. Contractor will provide quality assurance reports upon request, employ highly experienced and have monitored and controlled clean room conditions. Radiopharmaceuticals may be ordered up to 5:00 pm each day. Orders will be placed on an as needed basis buy an authorized VA employee. Delivery is required within 60 minutes of placing the order business hours, 0730-1700, per day 5 days per week, carrying a proper bill of lading and emergency response instruction with each shipment of radioactive material. If needed, the Contractor will provide unit-dose radiopharmaceuticals on Saturday and Sunday. Contractor will provide leukocyte and platelet labeling services, including weekends. If the volume warrants, leukocyte and platelet labeling will be available as same day service. Contractor will provide monthly invoices with complete itemized list of all products provided during the month, listed by prescription number, patient name, procedure, date and time and individual cost per item listed. Individual radiopharmaceuticals will be provided in unit dose form and will be delivered at the request of the VA. Contractor will provide 24-hour on-call service for emergency procedures.
