Description
Please provide a quote for the following itmes: Beverage Basics: For healthy Families - Length - 8 minutes; Producer: Lemon-Aid Films QTY 111 Portion control: Seeing the Healthy Way to Eat: Length - 22 minutes ; Producer: Human Relations Media QTY 111 Breakfast: Most Important meal of the Day : Length 20 minutes; Producer: Meridian Education Corporation QTY 111 This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the information in FAR Subpart 12.6, using Simplified Acquisition Procedures in accordance with FAR Part 13. This announcement constitutes the only solicitation; quotes are being requested, and a seperate solicitation will not be issued. PAPER COPIES OF THIS SOLICITATION WILL NOT BE AVAILABLE. The solicitation number is FNS-MC-004 and is being issued as an RFQ. The proposed contract is 100% set aside for small business concerns. This solicitation will result in a Firm Fixed Price (FFP) purchase order. The Government intends to evaluate quotes and award a purchase order using the simplified acquisition procedures of FAR Subpart 13.1. The Government shall select the vendor whose quote represents the lowest price. The Government reserves the right to not award a contract if a contract award is not in the best interest of the Government. INVOICES Invoices shall be submitted in an original and two copies to the Government office listed below. To constitute a proper invoice, the invoice must include the following information and/or attached documentation: (1) Name and address of the business concern, (2) Invoice number and invoice date; (3) Contract number, Delivery Order number, Purchase Order number, Task Order number, or other authorization for delivery of property or services actually delivered or rendered; (4) Description, unit price, extended price, and quantity of property and services actually delivered or rendered with supporting documentation (i.e. travel receipts, etc.); (5) Shipping and payment terms; (6) Name, title, phone number, and complete mailing address of responsible official to whom payment is to be sent; The Contractor shall submit an original and two (2) copies of invoices to the following designated payment office: U.S. Department of Agriculture Food and Nutrition Service Accounting Division 3101 Park Center Drive, Room 724 Alexandria, VA 22302 (7) Name, title, phone number, and mailing address of person to notify in the event of a defective invoice; and (8) Taxpayer Identification Number (TIN) or DUNS+4. Vendor(s) must be registered in the Central Contractor Registration (CCR) database to be considered for award. This is a solicitation for a commercially available off-the-shelf item. You shall select videos that have already been developed by your company to answer this solicitation. This is not a request to create and develop a DVD to satisfy the requirements listed above.
