- DHS HSAR 3052.209-70 Prohibition on Contracts with Corporate Expatriates (Jun 2006) Solicitation Number HSHQPD-09-Q-00018 Radiation Detection Equipment for the United States Department of Homeland Security (DHS) Plum Island Animal Disease Center (PIADC) 1. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented by additional information included in this posting. This posting constitutes the only solicitation for these supplies that will be issued and no other written solicitation will be issued. The Government intends to award a fixed-price purchase order in accordance with Simplified Acquisition Procedures (SAP) under FAR Part 13. 2. This solicitation, issued as a Request for Quotes (RFQ), has been assigned the reference number is HSHQPD-09-Q-00018. 3. This RFQ and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-26. 4. This solicitation is subject to full and open competition. 5. The suggested format for responses should include the Contract Line Item Numbers (CLINs), Descriptions, Quantities, Units of Measure, Unit Prices, and Amounts (Prices) are as follows: CLIN 0001 Description: First Responder Radiation Kits with the Following Components: Five (5) Range Survey Meters; Internal High Range G-M Detector; Alpha Beta Gamma Pancake Probe (G-M); 1" x 1" (one inch by one inch) Nal (TI) Gamma Scintillator; Check Source; Total Range 0-2000mR/Hr; and Rugged Airtight/Watertight Carrying Case Quantity: 15 Unit of Measure: Each Unit Price: ____________________ Amount (Price): ____________________ CLIN 0002 Description: First Responder Direct Reading (Pocket) Radiation Dosimeter, 0-200 mR Quantity: 50 Unit of Measure: Each Unit Price: ____________________ Amount (Price): ____________________ CLIN 0003 Description: Dosimeter Charger, SE Piezoelectric Charger 750-6 Quantity: 15 Unit of Measure: Each Unit Price: ____________________ Amount (Price): ____________________ CLIN 0004 Description: Rugged Airtight/Watertight Carrying Case for Dosimeter Chargers (Pelican 1500 Case with a Pick & Pluck Polyurethane Foam and Convoluted Lid) Quantity: 10 Unit of Measure: Each Unit Price: ____________________ Amount (Price): ____________________ TOTAL AMOUNT (PRICE): ____________________ Please include any manufacturer's part numbers with each item. 6. The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. 7. Offerors must submit the following with the quote: (a) Prices in accordance with Section 5; (b) Dun & Bradstreet Number (DUNS); (c) North American Industrial Classification System (NAICS) Code; (d) Business Size, Contact Name; (e) Contact E-mail Address; (f) Contact Telephone and Fax Number; (g) Complete Business Mailing Address; and (h) Addendum 1: HSAR 3052.209-70 Prohibition on Contracts with Corporate Expatriates Note: As a reminder, please download Attachment 1, check off the appropriate box, sign and date, and return this completed form with your quote. 8. The successful Offeror is hereby required to complete the Online Representation and Certification Application (ORCA) via http://orcha.bpn.gov upon notice of contract award. Please note that as of January 1, 2005, the FAR requires that Offerors complete Representations and Certifications ORCA at least annually. Solicitation & Contract Clauses: 9. The provision at 52.212-2, Evaluation - Commercial Items, applies to this acquisition. The Government will award a contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors shall be used to evaluate offers: Compliance with each of the descriptions of the requirement; Anticipated delivery date; and Price. Award will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factor. 10. 52.212-3 Offeror Representations and Certifications - Commercial Items. 11.
